Hodnocení nákladové efektivity totální endoprotézy ramenního kloubu

Abstract

Hlavním cílem diplomové práce je zhodnocení nákladové efektivity aloplastiky ramenního kloubu. Pro tento účel jsou vteoretické části zmapovány dostupné druhy endoprotéz a indikace kimplantaci. Pro hodnocení nákladové efektivity vpodmínkách České republiky je vybrána diagnóza traumatických tří a čtyř-úlomkových zlomenin proximálního konce humeruindikovaných kaloplastice ramenního kloubu. U této indikace je možné použít hemiartroplastiku a reverzní totální endoprotézu vzávislosti na rozsahu poškození a sohledem na celkový stav pacienta. Na obě hodnocené intervence jsou vyčísleny celkové náklady zpohledu poskytovatele zdravotní péče. Sohledem na výstupy zahraničních studií a dostupná data ze zdravotnické dokumentace pacientů jsou vybrána kritéria, která jsou dále použita do multikriteriální analýzy pro zhodnocení klinického efektu u každé hodnocené intervence. Závěrem je pomocí získaných údajů provedena analýza nákladové efektivity reverzní totální endoprotézy a hemiartroplastiky ramenního kloubu.

The mainobjective of this dissertation is to evaluate the cost-effectiveness of the shoulder joint arthroplasty. For this purpose, the available types of the endoprosthesis and indications for the implantation are mapped in the theoretical part. To assess cost -effectiveness in the Czech Republic is chosen diagnosis of traumatic three -and four-fragmentary fractures of the proximal end of the humerus indicated to the shoulder joint arthroplasty. For this indication can be used hemiarthroplasty and total reverse pro sthesis depending on the extent of damage with regard to the patient's general condition. O n both assessed interventions are quantified costs in terms of health care provider. With regard to the outcomes of foreign studies and available data from medical records of patients are selected the criteria that are also used in multi-criteria analysis for the clinical assessment of the effect for every intervention. Finally, the obtained data assist the cost -effectiveness analysisof the reverse total endoprosthesis and hemiarthroplasty of the shoulder joint
