První pomoc a ošetřování po pádu z výšky

Abstract

Bakalářská práce se zabývá první pomocí a ošetřováním poraněných po pádu z výšky a připraveností vybraných složek IZS na ošetření a transport těchto pacientů. V teoretické části práce je popsán pád z výšky a fyzikální děje, které se při tomto pohybu uplatňují, mechanismy poranění tkání v důsledku pádu a typy zranění způsobených pádem v závislosti na okolnostech a poloze těla v době pádu a při dopadu. Následují zásady první pomoci a ošetření zraněných po pádu z výšky se zaměřením na popis traumat, ke kterým obvykle při pádu z výšky a dopadu na podložku dochází. Dále jsou zde popsány legislativně stanovené podmínky a požadavky na personální obsazení a vybavení vybraných složek integrovaného záchranného systému. V praktické části práce je uvedeno personální obsazení a vybavení technickými a věcnými prostředky pro zajištění první pomoci, ošetření a transport pacienta po pádu z výšky u vybraných složek integrovaného záchranného systému Ústeckého kraje, kde šetření a sběr dat probíhaly. Dále jsou zde uvedeny poznatky z praxe získané dotazníkovým šetřením, přičemž respondenty byli zaměstnanci Zdravotnické záchranné služby Ústeckého kraje, rozhovory s vedoucí lékařkou letecké výjezdové skupiny Masarykovy nemocnice v Ústí nad Labem, třemi příslušníky Hasičského záchranného sboru Ústeckého kraje a náčelníkem Horské služby pro oblast Krušných hor. Dotazníkové šetření i rozhovory byly zaměřeny na zjištění údajů týkajících se personálního obsazení, používaného vybavení a připravenosti vybraných složek IZS na řešení první pomoci, ošetření a transport pacienta po pádu z výšky. Praktická část je doplněna informacemi získanými během stáží na základně LVS v Masarykově nemocnici Ústí nad Labem, na stanici HZS Ústeckého kraje v Žatci a na stanici horské služby v Tisé. Pro úplnost jsou zařazeny tři kazuistiky. Vyhodnocení získaných údajů je popsáno ve výsledcích. V závěru bakalářské práce je provedeno zhodnocení připravenosti vybraných složek IZS na ošetření a transport pacienta po pádu z výšky.

The bachelor thesis deals with the first aid and treatment of the patient after a fall from height and the readiness of selected constituents of the Integrated Rescue System to provide treatment and transport to such patients. The theoretical part describes the fall from height and the physical processes involved in it, the mechanics of tissue injuries as a result of the fall and the types of fall injuries as functions of the circumstances and the position of the body at the times of fall and impact. Then principles of first aid and treatment of the injured after a fall from height follow, with a focus on description of traumas usually occurring as a result of a fall from height and impact on the ground. Moreover, there are addressed the statutory conditions and requirements on the staffing and the equipment of selected constituents of the Integrated Rescue System. The practical part deals with the staffing and the technical and material equipment for the provision of first aid, treatment and transport to patients after a fall from height in selected constituents of the Integrated Rescue System of the Ústí nad Labem Region, where the research and data collection took place. Also included are real-life findings obtained through a survey the respondents of which are the employees of the Medical Rescue Service of the Ústí nad Labem Region, through talks with the head doctor of a medical air rescue team of the Masaryk Hospital in Ústí nad Labem, three members of the Fire Rescue Service of the Ústí nad Labem Region and the head of the Mountain Rescue Service for the Ore Mountains area. The survey and the talks were aimed at finding information about staffing, used equipment and readiness of selected constituents of the Integrated Rescue System to provide first aid, treatment and transport to a patient after a fall from height. The practical part is completed with information obtained during study stays at the air rescue team centre in the Masaryk Hospital in Ústí nad Labem, at the Fire Rescue Station of the Ústí nad Labem Region in Žatec and at the Mountain Rescue Station in Tisá. For the sake of completeness, three case histories are included. The interpretation of the obtained data is described among the results. The bachelor thesis concludes with the evaluation of the readiness of selected constituents of the Integrated Rescue System to provide treatment and transport to a patient after a fall from height.
