Porovnání připravenosti poskytovatelů zdravotnických záchranných služeb v ČR k řešení vybraných mimořádných událostí

Abstract

Moje diplomová práce se zabývá dvěma typy mimořádných událostí, kterými jsou AMOK - útok aktivního střelce podle typové činnosti složek integrovaného záchranného systému při společném zásahu (STČ 14/IZS) a vysoce nebezpečná nákaza, a to v souvislosti s připraveností vybraných poskytovatelů zdravotnické záchranné služby v České republice na jejich řešení. První část mé práce obsahuje úvod do problematiky, jsou zde vysvětleny základní pojmy týkající se oblastí zdravotnické záchranné služby, integrovaného záchranného systému, přípravy na mimořádné události a ochrany obyvatelstva, krizového řízení a kritické infrastruktury a také ochrany veřejného zdraví. V další části se již konkrétně zaměřuji na poskytovatele zdravotnické záchranné služby, zabývám se otázkou historického vývoje až do současnosti, včetně organizačního členění podle stávající platné právní úpravy. Také tu jsou vymezeny související právní předpisy a dokumenty. Věnuji se zde traumatologickému plánu, plánu krizové připravenosti subjektu kritické infrastruktury a souboru typových činností složek integrovaného záchranného systému při společném zásahu. Následující část se zabývá problematikou dvou konkrétních mimořádných událostí z hlediska jejich specifik a postupů zdravotnické záchranné služby. Praktická část diplomové práce je zaměřena na konkrétní průzkumné problémy, jejichž cílem bylo získání informací o připravenosti vybraných poskytovatelů zdravotnické záchranné služby k řešení výše uvedených typů mimořádných událostí, jejich vyhodnocení a porovnání získaných dat mezi sebou. Toto jsem prováděla pomocí strukturovaných rozhovorů vedených se zástupci managementu. Jednotlivá získaná data jsem vyhodnotila pomocí SWOT analýzy a grafů. Při vyhodnocování bylo přihlédnuto k výsledkům doplňující metody anonymních dotazníků pro členy výjezdových skupin zdravotnické záchranné služby, kterou jsem přidala během získávání informací od zástupců managementu. Tento vzorek odpovědí v anonymním dotazníku mi posloužil jako zpětná vazba k výsledkům. Poté jsem výsledná data jednotlivých poskytovatelů ZZS porovnala mezi sebou a tím ověřila stanovené hypotézy. Závěrem jsem navrhla směr možných řešení uvedených nedostatků v podobě zlepšení jednání a spolupráce s Policie České republiky při cvičeních, dále zavedení kurzů základů krizové psychologie, dále pak zvážení pořízení osobních ochranných pomůcek pro členy výjezdových skupin zdravotnické záchranné služby. V oblasti vysoce nebezpečných nákaz jsem navrhla, aby všichni poskytovatelé zdravotnické záchranné služby měli sestaven tzv. biohazard tým z vlastních zaměstnanců včetně řešení možností širšího využití tohoto specializovaného týmu.

My diploma thesis deals with two types of extraordinary events such as AMOK - the attack of an active shooter according to the type of activity of components of the Integrated rescue system under joint intervention (STČ 14 / IZS) and a highly dangerous infection, thus in connection with the preparedness of selected Medical Emergency Rescue Service providers in the Czech Republic to their solution. The first part of my work includes an introduction to the problematic, explaining the basic concepts in the field of Medical Emergency Rescue Service, the integrated rescue system, emergency preparedness and protection of the population, crisis management and critical infrastructure as well as protection of public health. In the next part, I am specifically focused on the provider of the Medical Emergency Rescue Services. I deal with the question of historical development up to the present, including organizational classification according to the current legal regulation. There are also defined related legal directives and documents. Here, I pursue a traumatological plan, a crisis preparedness plan subject to a critical infrastructure and a set of component activities of the integrated rescue system components in a joint intervention. The following section deals with the issue of two specific emergencies from the point of view of their specifics and procedures of the Medical Emergency Rescue Service. The practical part of the diploma thesis focuses on specific reconnaissance problems aimed at obtaining information about the preparedness of the selected Medical Emergency Rescue Service providers to solve the above-mentioned types of extraordinary events, their evaluation, and comparison of the acquired data among themselves. This was done by me through structured interviews with management representatives. The individually acquired data was evaluated using SWOT analysis and graphs. The evaluation has taken into account the results of anonymous questionnaires for Medical Emergency Rescue Service team members, which I added during my acquisition of information from management representatives. This sample of answers in the anonymous questionnaire served as a feedback to the results. After that, I compared the resulting data of individual ZZS providers with each other and thus verified the established hypotheses. In conclusion, I suggested the direction of possible solutions to the mentioned shortcomings in the form of improvement of negotiations and co-operation with the Czech Police during the exercises, the introduction of courses in crisis psychology fundamentals, and consideration of the purchase of personal protective equipment for members of the Emergency Medical Rescue Service. In the field of highly contagious diseases, I suggested that all Medical Emergency Rescue Service providers should have a so-called biohazard team from their own staff, including addressing the possibilities of a wider use of this specialized team.
