Blízká infračervená spektroskopie (NIRS) je neinvazivní spektroskopická metoda, která je nástrojem pro kontinuální měření nasycení tkání kyslíkem. Cílem práce je pomocí analýzy záznamu regionální cerebrální oxygenace extrémně nedonošených novorozenců měřených metodou NIRS navrhnout a otestovat metody, které umožní detekovat a identifikovat IVH. Kromě cerebrální oxygenace jsou dále sledovány hodnoty SpO2, vypočtené hodnoty koeficientu cerebrálního vstřebávaní kyslíku a jejích druhé diference v 1 a ve 20 minutových intervalech. Z navržených 6 hypotéz a založených podle nich metod, žádná neumožňuje za stanovených podmínek 100% identifikaci IVH skupiny pacientu. Dvě hypotézy založené na detekci IVH pomocí ploch mezi křivkou a stanovenou normou regionální cerebrální oxygenace a pomocí spektrálního rozloženi normalizovaných hodnot koeficientu cerebrálního vstřebávaní kyslíku nelze zamítnout. Nejlepší účinnost identifikace IVH ale ukazuje metoda zamítnuté hypotézy o závislost intraventrikulárního krvácení na variabilitě hodnot druhé diference po 20 minutovým intervalům koeficientu cerebrálního vstřebávaní kyslíku. Near infrared spectroscopy (NIRS) is a non-invasive spectroscopic method, which is used for continuous measurement of tissue saturation with oxygen. The aim of this work is to design and test methods for the detection and identification of IVH by analyzing the regional cerebral oxygenation records of extremely premature neonates measured by the NIRS. The following values of cerebral oxygenation were monitored: the SpO2 values, the calculated values of the cerebral oxygen absorption coefficient and its second difference at 1 and at 20 minutes intervals. None of the proposed 6 hypotheses and methods based thereon guarantees 100% identification of the IVH patient group under the specified conditions. Two hypotheses based on the detection of IVH based on the area between the curve and the established norm value of regional cerebral oxygenation standard, and on the spectral distribution of the normalized values of the cerebral oxygen absorption coefficient, cannot be rejected. However, the most efficient in the IVH identification was the method of the rejected hypothesis of intraventricular bleeding dependence on variability of the second differential by 20 minutes intervals of the cerebral oxygen absorption coefficient.
