Terapeutické využití laseru k ovlivnění bolesti v oblasti krční páteře a trapézových svalů

Abstract

Tato bakalářská práce se věnuje problematice hodnocení efektivity vysokovýkonného laseru v terapii bolesti v oblasti krční páteře. Kapitola současný stav se věnuje popisu anatomie páteře a kineziologie krční páteře. Dále pojednává o příčinách vzniku bolesti v oblasti krční páteře a bolesti obecně. Rovněž je v této kapitole uvedena charakteristika vysokovýkonného laseru, jeho účinky ve tkáních, indikace, kontraindikace a zásady bezpečné práce s laserem. Kapitola metodika zahrnuje popis vyšetřovacích a terapeutických postupů, které byly využity v praktické části. Při výzkumu byly využity k hodnocení efektivity vizuální analogová škála a Stupnice hodnocení efektu terapie, jejichž charakteristiky jsou zde rovněž popsány. Ve speciální části práce je uveden popis vstupních vyšetření pacientů a průběh terapie u jednotlivých skupin pacientů. U prvních dvou skupin byla aplikovaná laserová terapie v rámci komplexního rehabilitačního plánu v počtu šesti aplikací u první skupiny a deseti aplikací u druhé skupiny. Třetí skupina byla ošetřována manuálními technikami v rámci individuální fyzioterapie a byla zvolena jako kontrolní pro srovnání efektu laseru. Kapitola výsledků obsahuje hodnocení okamžitého a dlouhodobého účinku terapie zjištěného na základě subjektivních pocitů pacientů. V závěru je zhodnocen efekt laseroterapie a porovnán s výsledky dosaženými u kontrolní skupiny. Předmětem diskuze je srovnání efektu vysokovýkonné laseroterapie s poznatky z dalších publikovaných výzkumů v této oblasti.

This bachelor thesis focuses on the evaluation of the high-power laser effectiveness in the therapy of cervical spine pain. In the Theory section, spine anatomy and cervical spine kinesiology are described. Further, the etiology of cervical spine pain and pain in general are depicted. This section also provides characterization of the high-power laser, including lasers' effect on tissue, indications and contraindications, and principles of safe work with the laser. The Methods section comprises the description of the examination and therapy procedures which had been utilized in the practical part. Further, Visual analogical scale and Scale for evaluation of the therapy effectiveness used in this research are described. Specific sub-section provides description of the initial examination and the therapy procedures used for the treatment groups and control group. The Results section presents the patients' self-reported evaluation of both immediate and long-term effects. In the first and the second group laser therapy was applied as part of the comprehensive rehabilitation with six applications for the first group and ten applications for the second group. The third group received conventional simple segmental rehabilitation in the individual physiotherapy and was chosen as a control for evaluation of laser therapy effectiveness. The effectiveness of laser therapy in the treatment groups is assessed and compared with the control group' results. In the Discussion, the findings concerning effectiveness of high-power laser therapy for cervical spine pain are compared with findings from prior research.
