Terapie benigních onemocnění štítné žlázy otevřenými zářiči

Abstract

Terapie benigních onemocnění štítné žlázy je jednou z metod nukleární medicíny. I když jsou tyto metody oproti metodám diagnostickým v menšině, mají u některých pacientů významný přínos. Práce je rozdělena do dvou hlavních částí - teoretické a speciální. Teoretická část poskytuje informace o anatomii, fyziologii a patofyziologii štítné žlázy. Dále je stručně popsána nukleární medicína, jako klinický obor se zaměřením na terapeutickou oblast. Proto je v této části práce uveden mechanismus biologického účinku záření, typy a interakce ionizujícího záření. Výrazná část je věnována radiojodu 131I, jakožto radiofarmaku používanému pro léčbu onemocnění štítné žlázy a samotným terapeutickým postupům. V závěru teoretické části jsou uvedena vyšetření prováděná před terapií radiojodem 131I. Ke zhotovení metodiky speciální části práce jsem vycházela z Místních radiologických standardů dvou pražských fakultních nemocnic, která na oddělení nukleární medicíny provádějí léčbu radiojodem 131I. Tato část práce poskytuje informace o průběhu terapie benigních onemocnění štítné žlázy radiojodem 131I za ambulantního režimu a za hospitalizace v těchto nemocnicích. Dalším bodem speciální části jsou výsledky, kde porovnávám oba uvedené režimy. Následně jsou tato data také porovnána se současnými legislativními předpisy, u kterých vycházím z Národních radiologických standardů. Na závěr je uvedeno porovnání finančních nákladů obou režimů.

The therapy of benign thyroid disease is one part of nuclear medicine. Though the therapeutic methods are in a minority in comparison with diagnostic methods, they may have big impact on the patient. This paper is divided into two main parts - theoretical and special. Theoretical part gives information about anatomy, physiology and pathophysiology of thyroid gland. Then the nuclear medicine is briefly described as clinical field with focus on therapeutic methods. That is why there are described mechanism of biological impact of radiation, types and interaction of ionizing radiation. Important chapter is dedicated to radioiodine 131I, as radiopharmaceutical that is being used in the treatment of the thyroid disease and particular to the therapeutic procedures itself. Finally I mention the examinations that have been made before the 131I therapy. For preparation of the methodical part I used local radiological standards of two University Hospitals in Prague that have used 131I at their nuclear medicine department for treatment. This part of the paper yields information about the process of the treatment using 131I at outpatient and inpatient regime in those two hospitals. Next point of the special part are data, where I compare those two listed regimes. Data are then compared with current legislative regulations, which I gained from National radiological standards. In conclusion, financial aspects of both regimes are compared.
