Vliv hemodialýzy na množství protilátek proti viru hepatitidy B v séru pacientů s poruchou ledvin

Abstract

Bakalářská práce s názvem ,,Vliv hemodialýzy na množství protilátek proti viru hepatitidy B v séru pacientů s poruchou ledvin" je složena z teoretické a praktické části. V teoretické části je objasněna základní stavba a funkce ledvin. Také se zabývá rozdělením selhání ledvin a následně shrnuje jednotlivé možnosti náhrady funkce ledvin, kterými je peritoneální dialýza, hemodialýza a také transplantace ledvin. Největší pozornost je věnována objasnění principu hemodialýzy a jsou zde shrnuty její výhody a nevýhody. Dále popisuje původce hepatitidy B s podrobnější charakterizací jeho antigenní struktury a příznaků choroby, kterou způsobuje. Také možnosti léčby, prevence a očkování. V praktické části je popsán princip metody stanovení hladiny anti-HBs na biochemickém analyzátoru ARCHITECT. V souboru naměřených hodnot protilátek anti HBs, u pacientů z dialyzačního centra Fresenius Medical Care v Kladně, byla zkoumána hodnota hladin protilátek anti-HBs u skupiny pacientů před zahájením dialýzy, v jejím průběhu a u pacientů, kteří hepatitidu B prodělali již před zahájením hemodialýzy. Bylo zjištěno, že hladiny protilátek anti-HBs v séru pacientů v průběhu hemodialýzy mají klesající tendenci a to jak u pacientů očkovaných před hemodialýzou tak v jejím průběhu i u těch, kterým byl podán tzv. booster. Pacienti, kteří hepatiditu B prodělali již před zahájení hemodialýzy, mají naopak v pozorovaném období 23 měsíců hladinu anti-HBs bez velkých výkyvů a změn.

Bachelor thesis "The effects of haemodialysis on blood levels of antibodies against hepatitis B virus in patiens with renal impairment" is composed of a theoretical and practical part. The theoretical part of this bachelor thesis describes the anatomical structure and function of the kidneys. Further, thesis deals with the division of renal failure to acute and chronic and subsequently describes various possibilities of renal function replacement, such as peritonealdialysis, hemodialysis and renal transplantation. The greatest attention is paid to explanation of the principles of hemodialysis and in the thesis there are summarized advantages and disadvantages. The next part of the theoretical part of bachelor thesis is devoted to the causing agent of hepatitis B. This part contains information about the antigenic structure, description of the disease, treatment, prevention and vaccination. The practical part of the bachelor thesis describes the methods of antibodies anti-HBs determination, using the ARCHITECT biochemici analyzer. The levels of antibodies anti-HBs in patients from the Frenicus Medical Care dialysis center in Kladno are monitored. The levels of antibodies in patients efore hemodyalisis, undergoing hemodialysis are measured. The last group of patients are patients, who went through hepatitis B prior the hemodialysis. It was found, that the levels of anti-HBs in the serum of patients during the dialysis tend to decrease both in patients vaccinated before and during dialysis and those that were given ,,booster". Patients, who have got through hepatitis B before dialysis, maintained the level of antibodies without major fluctuations and changes during the 23 days - observed period.
