Vliv hysterektomie na hluboký stabilizační systém

Abstract

V bakalářské práci jsme se zabývali vlivem hysterektomie na hluboký stabilizační systém (dále jen HSS). Zaměřili jsme se na ženy,které se chystají podstoupit hysterektomii - laparoskopicky asistovanou vaginální hysterektomii (dále jen LAVH), totálně laparoskopickou hysterektomii (dále jen TLH) a abdominální hysterektomii (dále jen AHY). V teoretické části popisujeme druhy hysterektomie, které ženy podstoupily. Uvádíme indikace vedoucí k této gynekologické operaci a komplikace, které mohou nastat po jejím absolvování. Stručně seznamujeme s anatomickou stavbou pohlavní orgánové soustavy ženy a svalovou stavbou HSS a jeho významem. Popisujeme doporučenou fyzioterapii po hysterektomii. V metodologické části vysvětlujeme testy na HSS, kterými jsme zjišťovali jeho stav - testy dle Koláře, Trendelenburg - Duchennova zkouška a testy dle ,,Australské školy". Uvádíme metody cvičení, z kterých jsme čerpali inspiraci k sestavení cvičební jednotky pro posílení svalů HSS. V praktické části bylo cílem, pomocí výše uvedených testů a dotazníku, zjistit vliv hysterektomie na stav HSS. Výzkumu se zúčastnilo celkem 14 probandek. Před operací proběhlo vstupní vyšetření. Po operaci se uskutečnily dvě další vyšetření uvedených testů - kontrolní vyšetření (2 týdny po operaci) a výstupní vyšetření (8 týdnů po operaci). Dotazníky byly vyplňovány před a po operaci. Po operaci bylo probandkám doporučeno cvičit dle námi navržené cvičební jednotky a docházet jednou týdně na terapii. Celkem cvičilo a docházelo na terapii 7 probandek. Zbylých 7 probandek podstoupilo jen kontrolní a výstupní vyšetření. Poznatky z testů na HSS byly zpracovány textem. Výsledky dotazníku jsou popsány textem a vloženy do tabulek a grafů. Výzkum odhalil oslabení svalů HSS po hysterektomii a navrženou cvičební jednotku jsme shledali jako efektivní k jeho postupnému posilování.

In the bachelor thesis we followed up the impact of hysterectomy on the deep stabilization system (hereinafter HSS). We focused on women who are going to undergo a hysterectomy - laparoscopically assisted vaginal hysterectomy (hereinafter LAVH), total laparoscopic hysterectomy (hereinafter TLH) and abdominal hysterectomy (hereinafter AHY). In the theoretical part we describe types of hysterectomy that women have undergone. We introduce indications leading to this gynaecological surgery and complications which can occur after surgery. We briefly acquaint with anatomical structure of the woman's sexual organ system and muscular structure of the HSS and its importance. We describe recommended physical therapy after hysterectomy. In the methodological part we describe tests of HSS, determining its condition. Tests are tests by Kolář, Trendelenburg - Duchenne test and tests by "Australian school". We introduce methods, which have been an inspiration to set up an exercise plan for a stronger HSS. The aim of the practical part was to determine the impact of hysterectomy on the HSS with the help of mentioned tests. 14 probands participated in the research. Before surgery an entrance examination was carried out. Another two examinations were carried out after the surgery - a check up (after 2 weeks after the surgery) and the final examination (8 weeks after the surgery). The questionnaires were completed before and after surgery. The probands were recommended to exercise according to our suggested exercise unit and to attend physical therapy once a week. A total of 7 probands exercised and attended physical therapy. The remaining 7 participated only in the check up and the final examination. The tests findings were processed by text. The results of questionnaires are described by text and are put to tables and graphs. The research discovered lower muscle strength of HSS after hysterectomy and suggested exercise unit was found to be effective for its gradual strengthening.
