Radiační zátěž pacientů a personálu při intervenčních endovaskulárních výkonech na DSA

Abstract

Intervenční radiologie je metoda, která se dnes velmi vyvíjí, především zásluhou velkých úspěchů v léčbě velkého množství onemocnění. V posledních letech se zvyšuje počet případů indikovaných k angiografickému vyšetření. Při těchto vyšetření se mohou vyskytnout rizika, která jsou spojená s možným ozářením pacienta a personálu ionizujícím zářením. U některých vyšetření byly i hlášeny případy, kdy povrchová dávka ionizujícího záření dosahovala hraničních hodnot deterministických účinků. Ta to bakalářská práce zahrnuje studii 120 pacientů, kteří absolvovali různé angiografické intervenční endovaskulární výkony. Hodnotícím kritériem pro tu to práci byly: povrchová dávka produktu (DAP), povrchové dávky a efektivní dávka pacientů i pracovníků na angiografickém pracovišti radiodiagnostického oddělení Ústřední vojenské nemocnici - Vojenské fakultní nemocnici v Praze. Nejvýznamnějším kritériem byla hodnota DAP, kterou jsme získali pomocí DAP-metru. Z této hodnoty byly vypočítány další údaje. Na základě získaných a vypočítaných hodnot, se pokusíme v této práci zjistit náročnost některých intervenčních endovaskulárních výkonů z hlediska radiační zátěže pro pacienty a pracovníky.

Intervention radiology is a method that is greatly developing nowadays, mainly thanks to big successes in the numerous therapies of diseases. Lately, the number of cases indicated to the angiographic examination is increasing. With angiographic examinations, there are risks connected with possible ionizing radiation of the patient and the staff. There have been reported cases when the surface dose of the ionizing radiation reached values with deterministic effects. This bachelor's thesis includes the study of 100 patients, who underwent various endovascular procedures. The evaluation criterions for this work were: the surface dose of the product (DAP), surface doses, and the effective dose of the patients and the staff in the angiography section of the radiodiagnostic ward in the Central Military Hospital - Military Faculty Hospital in Prague. The most significant criterion was the value DAP (surface dose of the product), which was used to calculate other figures. On the basis of measured and calculated values and results, we will try to ascertain the demands of intervention procedures from the point of view of radiation strain for the patients and the staff. We will also try to understand the principle of radiation protection, and what can help decrease the dose.
