Sledování nákladů u chirurgických diagnóz

Abstract

Diplomová práce je zaměřena na sledování nákladů vybraných skupin chirurgických diagnóz. Hlavním cílem bylo vytvořit model sledování reálných nákladů v Oblastní nemocnici Kladno, jež by umožnil sledovat náklady až na úroveň jednotlivých případů hospitalizace a následně tyto náklady porovnat súhradami zdravotních pojišťoven. Zvolenými metodami pro dosažení stanoveného cíle byla analýza současného stavu sledování řízení nákladů nemocnic vČeské republice, kalkulace nákladů na léčbu vybraných diagnóz a úhrad zdravotních pojišťoven. Znákladových dat vybraného vzorku 30 pacientů byly počítány náklady jejich hospitalizace. Byly také zkoumány faktory, jež mohou náklady ovlivňovat. Délka hospitalizace a operační čas prokazují lineární závislost na nákladech. Zvýsledků je patrný signifikantní rozdíl mezi náklady na péči a úhradami za poskytnutou péči. Tyto rozdíly mohou být do značné míry dány rigiditou úhradového systému DRG či nepřesností vkalkulaci nákladů za daný případ hospitalizace. Na základě výsledků byla vytvořena doporučení na optimalizaci systému sledování nákladů vOblastní nemocnici Kladno.

The master thesis is focused on the monitoring of the chosen groups of the surgical diagnoses. The main objective was to the create a model of the cost management at the Regional hospital Kladno, which would allowed to follow up the costs on the level of every single patient and then compare these costs with the payments from health insurance companies. The methods chosen for achieving this goal was the analysis of the current situation of the hospitals cost management in the Czech republic, the c alculation of the patient's costs for the chosen diagnoses and the payments from the health insurance companies. The costs data from the sample of the 30 patients were used to calculate the costs on their treatment. The cost's influencing factors were exam ined also. Based on the results it is possible to conclude the linear dependency of the costs on the duration of the hospitalization and on the duration of the operation. The results are showing the distinctive difference between the costs of the care and the payments for this care. These differences could be set by the rigidity of the DRG payment system or by the inaccuracies made in the calculations of the costs for the case of a hospitalization.
