Fyzioterapeutická péče u pacienta s diagnózou periferní paréza nervus facialis

Abstract

Bakalářská práce je zaměřena na problematiku onemocnění periferní parézou lícního nervu. Toto onemocnění je v populaci poměrně časté a má dopad nejen na funkci mimických svalů, ale i na psychiku pacienta. Hlavními klinickými projevy onemocnění je stranová asymetrie obličeje, nehybnost jedné poloviny obličeje a s tím spojené další obtíže, mezi něž patří bolest v oblasti oka a ucha, špatná artikulace a poruchy chuti. Pacientka, která byla zdrojem dat pro speciální část této práce, prodělala jaterní virózu, provázenou bolestí hlavy, pro kterou byla hospitalizována na lůžkovém oddělení neurologie. Po absolvování všech vyšetření pro objasnění diagnózy byla její imunita oslabena a objevil se u ní herpetický výsev v oblasti levého ucha, který způsobil periferní parézu lícního nervu. Práce je dle obsahu rozdělena do několika kapitol. V úvodní části je poukázáno na problematiku onemocnění lícního nervu a jsou stanoveny cíle této práce. Cílem je ověření dosavadních fyzioterapeutických postupů, které se v současné době používají v léčbě daného onemocnění. Obecná část zahrnuje teoretické poznatky, týkající se anatomie nervové soustavy, anatomie svalů hlavy, etiologie, klinického obrazu a léčby. Metodologie práce obsahuje základní i speciální vyšetřovací a terapeutické postupy, které se v léčbě periferních paréz používají. Ve speciální části je kazuistika pacientky, jež zahrnuje anamnézu, provedená vyšetření a popis provedených terapií. Kapitola výsledky pojednává o dosažených pokrocích při terapii. V diskuzi je kriticky posouzen postup při terapii a další možnosti jejího vylepšení. Závěr stručně shrnuje dosažené výsledky terapie.

The Bachelor thesis is focused on the peripheral facial nerve palsy. This disorder is quite common within the population and impacts not only the functions of mimic nerves, but also the mental health of a patient. Main clinical symptoms of the disorder are facial lateral asymmetry, immobility of one facial half and other problems related, e.g. pain in the eye and ear area, impaired enunciation and dysguesia. The patient, whose data were a source for the special part of this thesis, has undergone liver flu accompanied with the headaches resulting in the admission in the neurological inpatient unit. Upon all the examinations necessary to clarify the diagnosis, her immunity was weakened and herpes infection occurred in the area of a left ear, causing in the peripheral facial nerve palsyThe thesis is separated into few chapters. The introductory part is devoted to the issue of facial nerve disorder and aims of the thesis are declared here. The aim is to verify existing physiotherapy procedures that are currently implemented within the disorder treatment. The general part includes theoretical outcomes concerning anatomy of nervous system, muscles of the head, etiology, clinical findings and treatment. Methodology contains basic and specialized examination and therapeutic procedures, which are being used within the treatment of the peripheral palsy. The specialized part includes the patient case study, including anamnesis, examinations performed and the description of the realised therapies. The chapter "results" describes achieved progress during the therapy. The discussion includes critical assessment of the therapeutic procedure and further possibilities of its improvement. The conclusion briefly summarises achieved results of the therapy.
