Přínos tympanometrického a otoskopického vyšetření u pacientů léčených v hyperbarické komoře

Abstract

Pacienti léčení hyperbarickou oxygenoterapii (dále HBO) jsou vystavováni změnám atmosférického tlaku, které působí na jejich organismus a mohou vyvolat řadu klinických obtíží. Jejich nejčastější potíže pramení z nedostatečné ventilace středoušní dutiny. Včasná a spolehlivá diagnostika omezené průchodnosti Eustachovy trubice může těmto komplikacím předejít. Tympanometrické a otoskopické vyšetření umožňuje nepřímo stav ventilace středou-ší posoudit. Na základě dohody s vedoucím lékařem oddělení bezpečnosti letů Ústavu leteckého zdravotnictví byl zpracován projekt a harmonogram klinického vyšetření v rámci běžného provozu zdravotnického zařízení. Na základě konzultací byly stanoveny nulové hypotézy jako východiska řešení. Ve spolupráci se zdravotnickým personálem zmíněného oddělení byl oběma metodami vyšetřen soubor 51 pacientů indikovaných k HBO. Pacienti se rovněž podrobili dotazníkovému šetření zaměřenému na anamnézu tolerance ke změnám atmosféric-kého tlaku a hodnocení léčebné kůry s důrazem na průběh první expozice HBO. Výsledky obou klinických vyšetření a dotazníkové akce byly podrobeny klinické a statistické analýze. Statistické šetření prokázalo statisticky významnou korelaci mezi všemi použitými metodami - tj. mezi tympanometrickým a otoskopickým vyšetřením a výsledky hodnocení subjektivních klinických obtíží v průběhu léčebné expozice. Na základě vyhodnocení získaných klinických výsledků a dat bylo vypracováno dopo-ručení pro klinickou praxi. Toto doporučení představuje algoritmus postupu pro posouzení způsobilosti k HBO. Jeho součástí jsou kritéria pro stanovení aktuální kontraindikace k léčebné kůře i postup v případě hraničních nálezů klinického vyšetření. Rutinní využití tympanometrického vyšetření, doplněného otoskopickým vyšetřením v případě hraničních a nevyhovujících tympanometrických nálezů umožňuje mnohem efektivnější rozhodování o zahájení léčby. Opakované vyšetření, v případě klinických obtíží při první expozici, spolu s vyhodnocením subjektivních stesků pacienta umožňuje rozhodování o eventuelním přeruše-ní či ukončení léčby. Navržený postup zejména snižuje počet pacientů vystavených klinickým obtížím v důsledku změn atmosférického tlaku. Současně přináší doložitelný organizační, diagnostický a finanční efekt.

Patients treated with HBO are exposed to the atmospheric pressure changes, that effect on their organism and can cause a number of clinical difficulties. The most common difficulties stem from the insufficient ventilation of the middle ear cavity. A well timed and reliable diagnostics of the limited clearness of the Eustachian tube allows to prevent from these complications. Tympanometric and othoscopic examination allows the indirect assessment of the middle ear cavity clearness. Based on the settlement with the head physician of the Department of Flight Safety of The Institute of Aviation Medicine a project and a time schedule of the clinical examination within the scope of medical facility's common duty were worked up. Based on the consultations the null hypotheses as the starting points of the solutions were worked up. In collaboration with the medical staff of the above mentioned department the group of 51 patients indicated for HBO was examined with both methods. The patients were resigned to the questionnaire investigation focussed on the tolerance to changes of the atmospheric pressure anamnesis and assessment of the treatment cure with the accent to the first HBO exposition process. The results of both of the clinical examinations and the questionnaire investigation were remitted to the clinical and statistical analysis. The statistical investigation showed statistical significant correlation among all of the used methods - i.e. among tympanometric and othoscopic examination and among the results of assessments of the subjective clinical difficulties during the treatment exposition. Based on the evaluation of retrieved clinical results and data a recommendation for clinical practice was worked out. This recommendation represents the proceeding algorithm for the assessment of the capability for HBO treatment. Criteria for evaluation of the actual contraindication for the treatment cure as well as the proceeding in the case of the terminal limiting findings of the clinical examination are part of the recommendation. The routine utilization of the tympanometric examination, supplemented by the othoscopic examination in the case of terminal and inconvenient tympanometric findings allows much more effective decision-making about the treatment start. The repeated examination in the case of clinical difficulties during the first exposition together with the evaluation of the patient's subjective complaints allows the decision-making about the possible interruption or termination of the treatment. The proposed proceeding principally lessens the number of patients exposed to the clinical difficulties in consequence of the atmospheric pressure changes. It bears sustainable organizational, diagnostic and financial effect concurrently.
