HeliOx v respirační péči

Abstract

Ke snížení dechové práce pacienta s obstrukcí dýchacích cest se provádíventilace směsí helia a kyslíku. Fyzikální vlastnosti helia umožňují snazší proudění plynu dýchacími cestami, to dovoluje pacientovi snazší dýchání oproti ventilaci vzduchem. V současné době se k ventilaci pacienta směsí helia a kyslíku používají otevřené okruhy, kdy je pacientovi přiváděna neustále čerstvá směs plynů, a tedy náklady na tuto ventilaci jsou vysoké. Aby se minimalizovala spotřeba helia, byl pro jeho aplikaci vyvinut systém s polouzavřeným okruhem a zpětným vdechováním, kdy se přivádí jen takové množství směsi, aby se pokryla spotřeba kyslíku organismem pacienta. Protože použití helioxu je v České republice problematikou novou, je nutné zanalyzovat dostupnost helioxu na českém trhu. K porovnání ekonomické náročnosti byly vytvořeny programy, kdy první program porovnává náklady na heliox odjednotlivých firem, dostupných na českém trhu a program druhý kalkuluje náklady na provoz, porovnáváním ekonomické náročnosti ventilace helioxem s otevřenýma polouzavřeným systémem. Dále byla vypracována rešeršní studie stávajícílegislativy související s použitím helioxu ve zdravotnických zařízeních.

To decrease work of breathing in patients with obstruction of the airways, ventilation by mixture of helium and oxygen (heliox) can be used. Physicalcharacteristics of helium enable an easier gas streaming through the airways allowing the patient an easier breathing compared to the ventilation with air. At present, only the open circuits are used for the helium and oxygen administration causing extremely high costs of the ventilation. In order to minimize helium demand, a system with semi-closed circuit and reinspiration was developed for its application. Only such amount of helium mixture is delivered into the circuit that covers the oxygen demand of the patient. Because usage of helium-oxygen mixture is a new approach in the Czech Republic, it is necessary to analyze the availability of heliox on the Czech market. To compare economic parameters, some special programswere created. The first one compares the costs if heliox offered by different companies acting on the Czech market. The second one calculates the operating costs comparing economic parameters of ventilation by heliox with the open and semi-closed system. Furthermore, a background research study of current legal aspects associated with the heliox usage in a medical environment was conducted.
