Možnosti rehabilitace pacienta po cévní mozkové příhodě v chronickém stadiu

Abstract

Bakalářská práce popisuje nynější pohled na léčbu a rehabilitaci spastické hemiparézy po cévní mozkové příhodě a srovnává jej s konvenčními postupy. Hlavní cíl je porovnat literaturu a efekt rehabilitace v klinické praxi použitím konvenčních i nových metod. Mezi konvenční metody patří Proprioceptivní neuromuskulární facilitace a Bobath koncept. Nové postupy, které byly aplikovány, jsou terapie intenzivním protažením, aplikace botulotoxinu typu A do vybraných spastických svalů a využití modelu konceptu Guided self-rehabilitation contract. Závěr by měl potvrdit vyšší efektivitu v léčbě spastické parézy novými postupy oproti konvenčním a měl by zdůraznit důležitost každodenní práce pacienta sama na sobě. Celkový efekt bude hodnocen, jak klinickým testováním, tak subjektivním pocitem pacienta. Z pohledu pacienta chceme zlepšit chůzi, rovnováhu a sebeobsluhu.

Bachelor's thesis describes the current view of the treatment and rehabilitation of spastic hemiparesis after a stroke, comparing it with conventional processes. The main aim of the thesis is to compare the effect of rehabilitation in clinical practice using conventional and new methods. Conventional methods include Proprioneuromuscular facilitation and the Bobath concept. New approaches that have been applied are intensive stretching therapy, application of botulinum toxin type A into selected spastic muscles and concept Guided self-rehabilitation contract. The results should confirm, that the treatment of spastic paresis with new metod sis more effective than the treatment with conventional process. Also it is important to emphasize the importance of the patient's daily excercise. The general effect will be evaluated by clinical testing, and the subjective feelings of the patient. From a patient perspective, we want to improve gait, balance and activities of daily living.
