Zvyšování efektivity operačního provozu

Abstract

Diplomová práce se zabývá zvyšováním efektivity operačního provozu na chirurgické klinice. Po provedení analýzy metod vhodných k implementaci v rámci operačního provozu byla zvolena, za účelem jeho zefektivnění, jako nejvhodnější metoda Lean Six Sigma. Při postupu vedoucího k zefektivnění operačního provozu byla dodržena filozofie metody Lean Six Sigma a její zásady, přičemž byla využita její metodika DMAIC s aplikací dílčích metod v jednotlivých fázích. Základem diplomové práce je analýza operačního procesu a vydefinování nejčastějších procesních nedostatků a kritických míst pro vznik neefektivit. Za výchozí stav procesu byla vzata současná situace na chirurgické klinice. Ve fázi definování bylo určeno jako nejkritičtější místo pro vznik neefektivit podávání antibiotické profylaxe. Měření proběhlo na vzorku operací z obecné chirurgie se zaměřením na střevní a rektální výkony. Parametry pro podávání antibiotické profylaxe byly hodnoceny v souladu s interním postupem schváleným vedením kliniky a antibiotickým centrem. Výsledky byly statisticky zpracovány, byla stanovena úroveň kvality sigma a následně byly zanalyzovány. Výsledkem diplomové práce je navržení interního postupu, který povede k adekvátnímu podávání antibiotické profylaxe, a tím ke zvýšení efektivity operačního provozu.

This thesis discusses increasing the efficiency of the operating room at a surgical clinic. After analysis of methods suitable for implementation in the operation room, a Lean Six Sigma method was chosen as the most suitable for making the operation room more efficient. During the process leading to more efficient operation room, Lean Six Sigma philosophy and its principles were followed, while its DMAIC methodology with application of partial methods in different stages was used. The basis of this thesis is an analysis of the operation room and defining the most common procedural deficiencies and critical points for the emergence of inefficiencies. The current situation at the surgical clinic was considered as the initial state of the process. In the stage of defining, antibiotic prophylaxis administration was identified as the most critical point for the emergence of inefficiencies. Measurement was carried out on a sample of general surgery operations with a focus on intestinal and rectal interventions. Parameters for administration of antibiotic prophylaxis were evaluated in accordance with the internal procedure approved by the clinic management and antibiotic centre. Results were statistically processed, sigma quality level was determined and subsequently analysed. The result of this thesis is a proposal of an internal process that will lead to an adequate administration of antibiotic prophylaxis, and thus increasing the efficiency of the operating room.
