Možnosti využití systému telemonitoringu v gynekologii a porodnictví

Abstract

Práce se zabývá prenatální péčí o rizikově gravidními pacientky, zejména s rizikem předčasného porodu. Cílem diplomové práce je porovnat telemonitorační systém s dosud používaným přístupem na gynekologicko-porodnických pracovištích. Nejdříve práce uvádí do problematiky, kde popisuje význam telemedicíny a telemonitoringu. Je zde rozebírána aktuálnost tématu a nechybí seznámení s hodnocenou intervencí. Literární rešerše zpracovává 16 odborných studií, z nichž jsou všechny uvedeny v práci a některé jsou rozebrány podrobněji. Zvolené metody pro dosažení stanoveného cíle byly literární rešerše pro získání ekonomicko-klinických efektů, metody vícekriteriálního rozhodování, analýza nákladů vybraných přístupů k prenatální péči a analýza nákladové efektivity. Zpracováním rešerše bylo vybráno 11 efektů. Během sběru dat byly získány charakteristiky gravidních pacientek s rizikem předčasného porodu z ambulantní péče. Sestaveny jsou dva soubory pacientek. První skupině byla poskytována standardní prenatální péče a pacientky z druhé skupiny obdržely také standardní péči, která je doplněna o dálkovou monitoraci děložní aktivity. Zmíněné efekty a veškeré náklady zpracovává analýza nákladové efektivity. V části diskuze je zpracována druhá analýza nákladové efektivity, která je v teoretické rovině aplikována na lůžkovou péči o gravidní pacientky.

The Thesis deals with prenatal care of patients with a high-risk pregnancy, especially of patients at high-risk for preterm labor. The aim of this Master?s Thesis is to compare the prenatal telemonitoring care with currently used attitude at obstetrics and gynecology consulting rooms. The first part of the Thesis introduces the issues and indicates importance of the telemedicine and telemonitoring. There are discussed topical problems and there is presentation of the intervention. The literature review processes 16 expert scientific studies and all of them are stated in the Thesis and some of them are processed in more detail. The methods chosen for achieving this goal were the literature review of the economic and clinical outputs, the methods of multi-criteria decision making, the analysis of the costs at the selected attitudes of prenatal care and the analysis of the cost-effectiveness. The 11 economic and clinical outputs were used in the study by the research of the clinical studies. Characteristics of the study population were used by data collection of patients at high-risk for preterm labor at the ambulatory care. There are compiled two groups of patients. The first group was provided a standard prenatal care and patients in the second group got also the standard care, but this care was accompanied by telemonitoring of uterine activity. The economic and clinical outputs and all the costs are processed in the analysis of the cost-effectivenes. The discussion of the Thesis contains the second analysis of the cost-effectiveness, which is processed in the theoretic level and the analysis is applied to an inpatient care of patients at high-risk for preterm labor.
