Ekonomické a klinické aspekty léčby karcinomu prostaty pomocí robotického systému daVinci

Abstract

Cílem práce je přinést komplexní pohled na robotickou operaci karcinomu prostaty, s důrazem na klinickou efektivitu této technologie v porovnání s otevřenou retropubickou radikální prostatektomií. Nejsou opomenuty ani etické a legislativní aspekty daných technologií při léčbě tohoto maligního onemocnění. Na základě klinických dat dvou souborů pacientů je provedeno srovnání parametrů a jejich statistická analýza. Mezi zahrnutými parametry jsou ty, které popisují kvalitu operace (operační čas, míra krevních ztrát), dále pooperační komplikace, onkologické charakteristiky (chirurgické okraje, PSA, biochemická recidiva) a funkční výsledky (kontinence moči, erektilní dysfunkce). Konečné hodnoty klinických efektů jsou stanoveny pomocí multikriteriálního hodnocení variant (2,38 pro robotickou operaci a 1,36 pro otevřenou klasickou operaci). Práce dále přináší hodnocení nákladů radikální prostatektomie z pohledu plátce zdravotní péče. Výpočet nákladovosti vychází přímo z dat Masarykova onkologického ústavu. Při porovnání samotné nákladovosti vykazuje roboticky asistovaná radikální prostatektomie oproti otevřené modalitě léčby vyšší náklady (118 156,70 Kč pro robotickou operaci oproti 85 678,80 Kč pro otevřenou prostatektomii). V závěru práce je provedena analýza nákladové efektivity, která porovnává získaná klinická a nákladová data obou metod radikální prostatektomie. Dle našich výsledků přináší robotická operace karcinomu prostaty v porovnání s otevřenou retropubickou prostatektomií vyšší efektivitu - poměr nákladů na jednotku klinického výstupu je u robotické operace nižší oproti otevřené radikální prostatektomii (poměr nákladů a efektu činí u robotické operace necelých 80 % stejné hodnoty pro otevřenou operaci).

The aim of the thesis is to provide a comprehensive view of a robotic surgery of a prostate cancer emphasizing the clinical effectiveness of this technology in comparison with open retropubic radical prostatectomy. Ethical as well as legal aspects of the technologies while treating the malignancy are taken into account. A comparison of parameters and their statistical analysis is made based on clinical data of two patients cohort. The parameters consist of those that describe the quality of the surgery (surgical time, quantity of blood loss), postoperative complications, oncologic characteristics (surgical margins, PSA, biochemical recurrence), and functional results (urinary continence, erectile dysfunction). The final values of the clinical effects are determined by the means of multi-criteria evaluation of alternatives (2,38 for a robotic surgery and 1,36 for a classical open surgery). The thesis also evaluates costs of radical prostatectomy from the perspective of a payer of the health care. Calculation of costs comes from the data taken by the Masaryk Institute of Oncology. When analysing the cost itself, the robotic assisted radical prostatectomy shows higher costs compared to the open modality treatment (118 156,70 CZC for the robotic surgerey and 85 678,80 CZC for the open prostatectomy). In the final part of the thesis, an analysis of cost effectiveness is realized. The analysis compares the gained clinical and cost data of both methods of the radical prostatectomy. Based on the results, the robotic surgery exhibits higher effectiveness as compared to the open retropubic prostatectomy - proportion of costs on a clinical outcomes unit when using the robotic surgery is lower than the costs of the open radical prostatectomy (the proportion of costs and effects in the robotic surgery is less than 80 % of the same value as for the open surgery).
