Fyzioterapie u pacienta s kombinovanou aortální vadou řešenou operačně — Rossovou metodou

Abstract

Předmětem této bakalářské práce je popsání fyzioterapeutické intervence u pacienta s kombinovanou aortální vadou, která byla řešena operačně Rossovou metodou. V teoretické části je popsána anatomie a fyziologie srdečních chlopní, také vrozené srdeční vady a choroby aorty. Byla zpracována problematika Rossovy operace její historie, výhody, nevýhody a indikace k této metodě. Je popsán rehabilitační postup, který se u pacientů po operaci srdce praktikuje. Metodika je zaměřena na popis vyšetřovacích metod a popis fyzioterapeutických postupů, které byly použity při práci s pacientem. Speciální část je zpracována formou kazuistiky. Byly provedeny vstupní vyšetření před operací a po operaci. Byla odebrána anamnéza a proveden komplexní kineziologický rozbor. Na základě vstupních vyšetření byl vytvořen krátkodobý a dlouhodobý terapeutický plán. Dále je popsán průběh jednotlivých terapeutických jednotek. Pacientovi byla vytvořena cvičební jednotka, která byla modifikována pode schopností pacienta. Výsledky obsahují výstupní kineziologický rozbor, je popsán efekt fyzioterapie, je popsán průběh echokardiografických vyšetření a můžeme zde nalézt grafické znázornění měnící se ejekční frakce levé komory během spolupráce s pacientem.

The subject of this bachelor thesis is to describe the physiotherapeutic intervention in a patient with combined aortic defect, which was solved by the Ross method. The theoretical part describes the anatomy and physiology of the heart valves, also congenital heart defects and aortic diseases. The problems of the Ross operation - its history, advantages, disadvantages and indications for this method were discussed. The rehabilitation procedure practiced in patients after heart surgery is described. The methodology is focused on the description of examination methods and description of physiotherapeutic procedures that were used in working with the patient. The special part is treated in the form of a case report. Preoperative and postoperative initial examinations were performed. Medical history was taken and a comprehensive kinesiological analysis was performed. Based on the initial examinations, a short-term and long-term therapeutic plan was developed. The course of the individual therapeutic units is also described. An exercise unit was created for the patient and modified according to the patient's abilities. The results include an output kinesiological analysis, the effect of physiotherapy is described, the course of echocardiographic examinations is described and a graphical representation of the changing left ventricular ejection fraction during the cooperation with the patient can be found.
