Specifika ošetřovatelské péče při aplikaci eliminačních metod v akutní intenzivní péči

Abstract

Bakalářská práce je zaměřena na specifika ošetřovatelské péče při aplikaci eliminačních metod u akutního renálního poškození v intenzivní péči. Cílem práce je celkové zhodnocení poskytované ošetřovatelské péče při aplikaci kontinuální venovenózní hemodialýzy a komparace této péče s nejnovějšími poznatky. Dalším cílem je pak vytvoření optimálního postupu pro uvedený typ ošetřovatelské péče a v neposlední řadě prezentace náročnosti ošetřování pacienta v kritickém stavu na anesteziologicko-resuscitačním oddělení (ARO). V teoretické části jsou v návaznostech vysvětleny pojmy související s akutním renálním poškozením. Je stručně popsána obecná anatomie a fyziologie ledvin, následuje deskripce akutního renálního poškození, jeho druhy, diagnostika, rizikové faktory pro rozvoj a možnosti léčby. Zahrnuta je charakteristika eliminační léčby, kontinuální venovenózní hemodialýzy, cévních přístupů, antikoagulační léčby a komplexní ošetřovatelské péče při aplikaci kontinuální venovenózní hemodialýzy. Praktická část je zpracována kvalitativní formou pomocí případových studií, ve kterých jsou znázorněny jednotlivé postupy ošetřovatelské péče při aplikaci kontinuální venovenózní hemodialýzy. V rámci výsledků při řešení výše uvedené problematiky se ukazuje, že správně poskytovaná ošetřovatelská péče má klíčovou roli nejen v přímé péči při aplikaci eliminačních metod, ale je důležitá zejména pro prevenci komplikací a že pro bezpečnou a efektivní aplikaci těchto metod je nezbytná důkladná edukace angažovaných zdravotnických pracovníků.

This bachelor´s thesis focuses on the specifics of nursing care with the application of eliminating methods in acute renale damage in intesive care. The target of the thesis is the overall evaluation of the nursing care provided during the applied continuous venovenous hemodialysis and the comparison of this care with the latest findings. Another goal is to create an optimal procedure for the mentioned type of nursing care and, last but not least, to present the difficulty of treating a patient in a critical condition at the anesthesia-resuscitation department of the ICU. In the theoretical part, the concepts related to acute renal damage are explained in context. The general anatomy and physiology of the kidneys is described, followed by a description of acute renal damage, the types, diagnosis, risk factors for development and treatment options. The characteristics of elimination treatment, continuous venovenous hemodialysis, vascular accesses, anticoagulant treatment and the complex nursing care in the application of continuous venovenous hemodialysis are included. The practical part is processed in a qualitative form with the help of case studies, in which the individual procedures of nursing care during the application of continous venovenous hemodialysis are shown. Within the findings of above mentioned challenges we can clearly see, that correctly applied care represents the key to success not only in the direct care with applied eliminating methods, but it is crucial in prevention of complications. In addition, for safe and effective application of these methods, the thorough education of all involved health care workers is absolutely necessary.
