Vliv aplikace McKenzie metody u derangemenet syndromu na parametry chůze

Abstract

Incidence bolestí dolní části zad konstantě stoupá v průběhu již desítek let, a tak tyto bolesti představují velký podnět pro vědecké zkoumání. Samotné poškození meziobratlových disků ať už ve formě proturze, sequestru či herniace je sporným tématem dnešní doby zejména pro rozpor v jejich nálezu na zobrazovacích metodách a klinickém obrazu. Metodou cílenou na tyto obtíže je koncept McKenzie, který je klinicky velmi dobře hodnocen pro svůj rychlý efekt. Předmětem diplomové práce je tak posouzení změny bezprostředně po jedné terapii u pacientů s těmito obtížemi. Samotným cílem práce je tuto změnu zkoumat z objektivního hlediska, proto jsou pro zhodnocení vybrány parametry chůze, a nikoliv pouze subjektivní dotazníky, které McKenzie metoda využívá. Výzkum je proveden na souboru 20 probandů. Pro objektivizaci parametrů chůze je využit zdravotnický prostředek Rehagait. Analýza chůze je tak provedena vždy bezprostředně před a po terapii McKenzie metodou. Pro vyhodnocení jsou dle dostupné evidence zvoleny parametry – délka kroku, trvání kroku, rychlost chůze a kadence. Ty jsou zpracovány statisticky a pomocí grafů. Změna parametrů chůze bezprostředně po McKenzie terapii se v rámci výzkumu této práce prokázala pouze na rychlosti a kadenci chůze. Diskuze práce se tak v závěru odklání od tradovaných teorií metody McKenzie a naopak se snaží přiblížit možné, byť i kontroverzní teorie a principy vzhledem k dosaženým výsledkům studií posledních let.

The incidence of lower back pain has been steadily increasing over the decades, and so these pains represent a great stimulus for scientific investigation. Damage to the intervertebral discs themselves, whether in the form of protrusion, sequestration, or herniation, is a controversial topic nowadays, especially because of the contradiction in their findings on imaging methods and clinical picture. The method aimed at these difficulties is the McKenzie concept, which is clinically very well evaluated for its quick effect. The subject of this thesis is the assessment of the immediate change after one treatment in patients with these difficulties. The very aim of the work is to examine this change from an objective point of view, therefore, gait parameters are chosen for evaluation, not just the subjective questionnaires that the McKenzie method uses. The research is conducted on a set of 20 subjects. The Rehagait medical device is used to objectify the gait parameters. The gait analysis is always performed immediately before and after McKenzie method therapy. The parameters – stride length, strike duration, velocity and cadence – are selected for evaluation according to the available evidence. These are processed statistically and through the use of graphs. The change in gait parameters immediately after McKenzie therapy has been shown in the research of this work only on gait speed and cadence. The discussion of the thesis departs from the traditional theories of the McKenzie method and, on the contrary, attempts to bring possible, even controversial, theories and principles taking into consideration the results obtained from studies in recent years.
