Nekrotizující infekce měkkých tkání a možnosti hyperbarické léčby

Abstract

Nekrotizující infekce měkkých tkání jsou závažná a život ohrožující onemocnění, která při neposkytnutí rychlé a multioborové léčebné strategie mohou končit amputací končetiny či smrtí pacienta. Za vysoce efektivní adjuvantní léčebnou metodou je považována hyperbarická oxygenoterapie. Teoretická část se zabývá charakteristikou, diagnostikou, klasifikací, etiologií a klinickými příznaky nekrotizujících infekcí měkkých tkání. Následně je popsán vývoj hyperbarické medicíny ve světě a aktuální postavení v České republice. Zmíněny jsou základní fyzikální a fyziologické aspekty této terapie, její indikace a vliv na organismus. Praktická část je zpracována formou kazuistik pěti pacientů, kteří byli hospitalizováni na anesteziologicko-resuscitačním oddělení Nemocnice Na Homolce, kde podstupovali hyperbarickou oxygenoterapii. Aplikované postupy léčby byly porovnány s doporučenými a byl zhodnocen stav pacientů prostřednictvím vybraných zánětlivých markerů a katecholaminové podpory.

Necrotizing soft tissue infections are severe and life-threatening diseases that, if not treated promptly with a multidisciplinary approach, can result in limb amputation or patient death. Hyperbaric oxygen therapy is considered a highly effective adjuvant treatment method. The theoretical part deals with the characteristics, diagnosis, classification, etiology, and clinical symptoms of necrotizing soft tissue infections. The development of hyperbaric medicine worldwide and its current position in the Czech Republic are then described. Basic physical and physiological aspects of this therapy, its indications, and its effects on the body are mentioned. The practical part is presented in the form of case studies of five patients who were hospitalized in the anesthesiology and resuscitation department of Na Homolce Hospital, where they underwent hyperbaric oxygen therapy. The applied treatment procedures were compared with recommended ones, and the patients' condition was evaluated through selected inflammatory markers and catecholamine support.
