Zajištění dýchacích cest obézních a morbidně obézních pacientů

Abstract

Bakalářská práce se zabývá problematikou obezity a s tím spojenou problematikou ohledně zajištění dýchacích cest. Hlavní cíl bylo zjištění, zda obezita negativně ovlivňuje proces endotracheální intubace. Vzhledem k tomu, že obezita představuje celosvětový problém a výskyt těchto pacientů ve zdravotnictví stále roste. Práce pojednává o velikosti rizika a přístupu jednotlivých anesteziologů k problematice obezity v případě zajištění dýchacích cest endotracheální cestou. Teoretická část pojednává o obezitě, popisuje její vznik, výskyt i predispozice. Uvádí historický pohled na obezitu a vývoj jejího definování. Následně se zaměřuje na její léčbu a prevenci. Zmíněná je i obezita u dětí a její případná léčba. Dále se zaměřuje na způsoby zajištění dýchacích cest jak v přednemocniční neodkladné péči, tak v případě anesteziologie a představuje základní poznatky těchto postupů. Popisuje základní poznatky a postupy ve směru zajištění dýchacích cest. Praktická část se zabývá výzkumem, jak velké komplikace obezita představuje pro anesteziologa, který potřebuje endotracheálně zajistit dýchací cesty obézního pacienta. Výzkum byl proveden na základě náhodného výběru 15 obézních a morbidně obézních pacientů a následného rozboru anesteziologické zprávy. Na základě nasbíraných dat jsou pak prezentovány výsledky, které jsou následně porovnány s podobnými šetřeními a studiemi ze světa.

The bachelor thesis deals with the issue of obesity and the associated problem of securing airways. The main objective was to determine whether, in today's world where obesity is still on the rise and is the biggest lifestyle disease, it poses any problems in terms of securing airways. The thesis discusses the risk level and approach of individual anesthesiologists to the issue of obesity in securing airways via endotracheal intubation. The theoretical part discusses obesity, describing its origin, prevalence, and predisposition. It provides a historical overview of obesity and the development of its definition. It then focuses on its treatment and prevention. It also mentions obesity in children and its potential treatment. Furthermore, it focuses on ways of securing airways, both in pre-hospital emergency care and in anesthesia, and presents the basic principles of these procedures. The practical part deals with research on the extent to which obesity poses complications for an anesthesiologist who needs to secure airways via endotracheal intubation in an obese patient. The research was conducted based on a random selection of 15 obese and morbidly obese patients and subsequent analysis of their anesthesia reports. The collected data is then presented, and the results are compared with similar investigations and studies from around the world.
