Ruptura rotátorové manžety (RM) je jedno z nejběžnějších poranění ramenního kloubu, které vzniká z důvodu degenerativních procesů, a proto je tato diagnóza zejména spojována s vyšším věkem. Panuje nejednoznačnost mezi volbou nejlepšího léčebného postupu pro toto poranění. Tato práce porovnávala dvě artroskopicky prováděné techniky sutury šlachy, single row (SR) a double row (DR) suturu. Cílem diplomové práce bylo zhodnotit nákladovou efektivitu double row sutury oproti single row. Pro provedení analýzy nákladové efektivity (CEA) byl sestaven model rozhodovacího stromu. V modelu byly uvažovány přímé náklady z perspektivy plátce zdravotní péče. Náklady byly stanoveny dle systému CZ-DRG a dle balíčku výkonů pro každou techniku zvlášť. Efekty byly posuzovány ve formě ASES skóre pro dvě podskupiny pacientů (ruptura <3 cm a >3 cm), a spolu s pravděpodobnostmi byly odvozeny z literární rešerše. Byla provedena jednocestná analýza sensitivity. Vyhodnotilo se celkem 6 scénářů. Hodnoty ICER vyšly v rozmezí 31,21–1 352,42 Kč/bod ASES skóre. Inkrementální náklady na DR suturu se pohybovaly v rozmezí 156,32–7 164,05 Kč. Inkrementální efekt generovaný DR suturou byl 5,30 bodů (ruptura <3 cm) a 7,05 bodů (ruptura >3 cm) ASES skóre. Výsledky CEA ani analýzy sensitivity nepotvrdily dominanci žádné z porovnávaných technik. DR sutura se v ploše nákladové efektivity ve všech případech umístila v pravém horním kvadrantu, což značí, že je dražší, ale i efektivnější oproti SR sutuře, zejména u podskupiny s rupturou RM >3 cm. Nicméně o přijetí DR sutury rozhoduje hranice ochoty platit, která není pro ASES skóre v ČR stanovena. Rotator cuff tear (RCT) is one of the most common shoulder injuries that occur due to degenerative processes, and therefore this diagnosis is mainly associated with older age. There is ambiguity between the choice of the best treatment approach for this injury. This paper compares two arthroscopically performed tendon suture techniques, single-row (SR) and double-row (DR) suture. This thesis aims to evaluate the cost-effectiveness of double row versus single row suture. A decision tree model was constructed to perform a cost-effectiveness analysis (CEA). The model considered direct costs from the perspective of the health care payer. Costs were determined according to the CZ-DRG system and according to the package of procedures for each technique separately. Effects were assessed in the form of ASES score for two subgroups of patients (tear <3 cm and >3 cm), and together with probabilities were derived from a literature search. One-way sensitivity analysis was performed. A total of 6 scenarios were evaluated. The ICER values came out in the range of 31.21–1 352.42 CZK/point ASES score. The incremental cost of DR suture ranged from 156.32–7 164.05 CZK. The incremental effect generated by DR suture was 5.30 points (tear <3 cm) and 7.05 points (tear >3 cm) ASES score. The results of CEA and sensitivity analysis did not confirm the dominance of any of the compared techniques. DR suture ranked in the right upper quadrant in the cost-effectiveness plane in all cases, indicating that it is more expensive but also more effective compared with SR suture, especially in the subgroup with RM tear >3 cm. However, the acceptance of DR suture is determined by the willingness-to-pay threshold, which is not established for the ASES score in the Czech Republic.
