Vliv vybraných faktorů na dynamiku sycení velkých cév srdce kontrastní látkou během CT angiografie při použití techniky test bolus

Abstract

Bakalářská práce pojednává o různých biologických a technických faktorech, které mohou ovlivnit dynamiku kontrastní látky podané během CT angiografie. Teoretická část popisuje základy CT angiografie velkých cév srdce a nynější trendy v jejich zobrazení. Popisuje kontrastní látky používané při CT angiografii, možnosti jejich podání a monitorace její dynamiky. V poslední řadě práce popisuje stručný základ radiační ochrany. V praktické části jsme ověřili poznatky z teoretické části s daty nasbíraných na pracovišti Základna radiodiagnostiky a intervenční radiologie, Institutu klinické a experimentální medicíny. V práci jsme poté porovnávali pomocí regresní analýzy rozdílnosti mezi pacienty s různým BMI, věkem a pacienty s hypertenzí a ICHS. Použili jsme regresní analýzu s ohledem na pacientovo BMI, věk, a také na příkon jódu. Data pacientů s ICHS a hypertenzí jsme porovnali s pacienty, kteří onemocnění diagnostikované nemají. Výsledkem práce jsou tak tabulky, grafy a regresní modely pro jednotlivé cévy. Přišli jsme tedy na vztahy mezi časy a hodnotami peaků k věku a BMI. U pacientů s vyšším BMI jsou časy delší a hodnoty nižší. S přibývajícím věkem pacienta je pak dynamika pomalejší, časy jsou delší a hodnoty peaků jsou vyšší. Diskuse blíže pojednává o samotných faktorech a nabízí vysvětlení výsledků. Data mohou být dále použita pro bližší analýzy a navazující práce.

The bachelor thesis deals with various biological and technical factors that can affect the dynamics of contrast agents administered during CT angiography. The theoretical part describes the foundations of CT angiography of large blood vessels and the current trends in their imaging. It describes the contrast agents used in CT angiography, the possibilities of their administration and monitoring its dynamics. Last but not least, the work describes a brief basis for radiation protection. In the practical part we verified the knowledge of the theoretical part with the data collected in the workplace of the Base of radiodiagnostics and intervention radiology, The institute of clinical and experimental medicine. We then compared the differences between patients with different BMI, age and patients with hypertension and IHD. We used regression analysis with respect to patient BMI, age, and iodine injection rate. We compared data from patients with CHD and hypertension with patients with and without these diseases diagnosed. The result of the work is tables, graphs, and regression models for individual blood vessels. We have discovered the relationships between the times and values of contrast enhancement peaks and age and BMI. In patients with higher BMI, the times are longer, and the values are lower. With increasing age of the patient, the dynamics are slower, times are longer and peak values are higher. The discussion deals with the factors themselves and offers explanations of the results. The data can be used further for more detailed analyzes and follow-up work.
