Využití autologních transfuzí ve FN Motol

Abstract

Bakalářská práce pojednává o využití autologních transfuzní ve Fakultní nemocnici Motol. V teoretické části se zabývá nejprve krví a vyšetřením krve a krevních složek dárce, následně definicí autotransfuze a stručnou historií, obecnými výhodami a nevýhodami autologních odběrů a indikacemi k jejich provedení, a nakonec pojednává o jednotlivých druzích autotransfuzí. Praktická část je zaměřena na předoperační sběr plné krve. Popisuje postup při indikaci k odběru, předtransfuzní vyšetření pacienta-dárce, samotný postup při odběru autologní krve a stručně i kontrolu jakosti autologních odběrů krve. Výsledky práce naznačují nárůst počtu odebraných autologních transfuzí za roky 2019-2021. Během těchto tří let bylo odebráno 733 transfuzních jednotek autologní krve. Autologní transfuze jsou stále indikovány převážně při ortopedických operacích a urologických zákrocích. Autotransfuze jsou mnohdy přijatelnější pro pacienty, kteří mají osobní problém s příjmem alogenní krve, případně jsou řešením např. při výskytu vzácných protilátek (např. anti-HFA - High frequency antigen) a nemožnosti nalezení vhodného dárce krve.

The bachelor’s thesis discusses the use of autologous transfusions in the Motol University Hospital. In the theoretical part, it deals first with blood and the examination of blood and blood components of the donor, then with the definition of autotransfusion and a brief history, general advantages and disadvantages of autologous sampling and indications for their performance, and finally discusses individual types of autotransfusion. The practical part is focused on the pre-operative collection of whole blood. It describes the procedure for the indication for collection, the pre-transfusion examination of the patient-donor, the procedure itself for the collection of autologous blood and, briefly, the quality control of autologous blood collections. The results of the work indicate an increase in the number of collected autologous transfusions for the years 2019-2021. During these three years, 733 transfused units of autologous blood were collected. Autologous transfusions are still indicated mainly for orthopedic surgery and urological procedures. Autotransfusions are often more acceptable for patients who have a personal problem with receiving allogeneic blood, or are a solution, for example, in the presence of rare antibodies (e. g. anti-HFA - High frequency antigen) and the impossibility of finding a suitable blood donor.
