Vliv manuálních technik na rozvoj páteře u pacientů s ankylozující spondylartritidou (axSpA) - pilotní studie.

Abstract

V této bakalářské práci je zpracována tématika onemocnění axiální spondylartritidy a možnost jejího ovlivnění pomocí manuální terapie. Teoretická část uvádí cíle a problematiku axiální spondylartritidy. Cílem je zlepšení subjektivní tuhosti a bolesti a mobility páteře. V úvodní části je prozkoumána základní definice spondylartritid a axiální spondylartritidy. Je zde uvedena stručná historie jak onemocnění samotného, tak i historický vývoj v jeho diagnostice. Popsán byl typický klinický obraz a etiologie onemocnění. Další část je poté věnována možnostem léčby, a to jak konzervativní terapie, mezi kterou patří i manuální terapie, tak je uvedena léčba farmakologická či operační. V metodologické části jsou uvedeny jednotlivé postupy, kterých bylo využíváno k provedení terapií a k hodnocení dosažených výsledků. V terapiích bylo využito manuálních technik a k hodnocení byly využity speciální indexi BASDAI, BASFI, BASMI a měření mobility páteře. Ve speciální části jsou zpracovány jednotlivé vstupní a výstupní vyšetření pacientů, a je zde uveden průběh terapií. V části výsledky jsou uvedeny tabulky, ve kterých jsou porovnány výsledky ze vstupního a výstupního vyšetření pacientů. Zároveň jsou zde porovnávány výsledky z indexů, které byli použity pro subjektivní hodnocení terapie pacientem. Z výsledků lze vyčíst, že došlo k předpokládanému zlepšení jak v rozvoji páteře, tak v subjektivním hodnocení pacientů. V diskusi jsou porovnávány výsledky dosažené v této práci a výsledky dosažené v jiných výzkumech a pracích zabývajících se podobnou tématikou. Závěrem je uvedeno, že manuální techniky jsou vhodným typem léčby pro pacienty s axSpA.

This bachelor thesis addresses the topic of axial spondylarthritis and the possibility of influencing it by manual therapy. The theoretical part presents the aims and problems of axial spondylarthritis. The aim is to improve subjective stiffness and pain and mobility of the spine. In the introductory part, the basic definition of spondylarthritis and axial spondylarthritis are examined. A brief history of both the disease itself is presented, as well as historical developments in its diagnosis. The typical clinical picture and the etiology of the disease is described. The next section is then devoted to treatment options, both conservative therapy, which includes manual therapy, and pharmacological or surgical treatment. In the methodological section, the different procedures used to perform the therapies and to evaluate the results obtained are presented. Manual techniques were used for therapies, and for evaluation special indexes BASDAI, BASFI, BASMI and spinal mobility measurements were used. In the special section, the individual initial and outcome examinations of the patients are elaborated, and the procedure is presented. In the results section, individual charts comparing the results from the initial and outcome examinations of the patients are presented. At the same time, the results from the indexes, which were used for the subjective evaluation of the therapy by the patient, are compared. From the results we can see that there was the expected improvement in both spinal development and subjective patient assessments. The discussion compares the results obtained in the present study with those obtained in other studies and theses dealing with similar topics. In conclusion is stated, that manual techniques are an adequate type of treatment for patients with axSpA.
