Měření velikosti duhovko-rohovkového úhlu

Abstract

Teoretická část se zaobírá patofyziologií přední komory a iridokorneálním úhlem v souvislosti s glaukomovým onemocněním, součástí je též principiální objasnění přístrojových metod pro vyšetření přední komory oka. V experimentální části byla provedena komparace dvou přístrojových metod. Větší koeficient spolehlivosti byl zjištěn při měření iridokorneálního úhlu na manuálním OCT vyšetření s předně-segmentovým modulem. Byla prokázána závislost jednotlivých biometrických parametrů oka na velikost D-RÚ: velmi silná pozitivní korelace pro ACD, střední pozitivní korelace pro AL, WtW a PD. Střední negativní korelace pro LT. Nevýznamný vliv na velikosti úhlu mají ostatní parametry: sagitta, tloušťka a keratometrie rohovky. Průměrná hodnota absolutního rozdílu mezi vypočítanou velikostí iridokorneálního úhlu na základě výstupního vzorce regresivní analýzy (v závislosti na biometrických parametrech oka) a reálné hodnoty naměřené na OCT činila 2,43°, přičemž rozdíl mezi daty nebyl statisticky významný.

The theoretical part deals with the pathophysiology of the anterior chamber and the iridocorneal angle in relation to glaucoma disease, it also includes a principal explanation of the instrumental methods for the examination of the anterior chamber of the eye. In the experimental part, a comparison of two instrumental methods was performed. A higher reliability coefficient was found for the measurement of the iridocorneal angle on manual OCT examination with the anterior-segment module. The dependence of individual biometric parameters of the eye on the size of the D-RU was demonstrated: very strong positive correlation for ACD, moderate positive correlation for AL, WtW and PD. Medium negative correlation for LT. Insignificant influence of other parameters on the size of the angle: sagitta, corneal thickness and keratometry. The mean value of the absolute difference between the calculated iridocorneal angle magnitude based on the output formula of the regression analysis (depending on the biometric parameters of the eye) and the actual value measured on OCT was 2.43°, and the difference between the data was not statistically significant.
