Vliv hemimelie horní končetiny na vývoj dítěte v batolecím věku

Abstract

Bakalářská práce je zpracována jako kazuistické šetření pacientky s vrozenou vývojovou vadou horní končetiny klasifikovaného pod diagnózou Q799, jako hemimelie horní končetiny. V rámci práce je popsána fyzioterapeutická intervence u pacientky batolecího věku. Práce je rozdělena na část teoretickou, metodickou a speciální. Teoretická část je rozdělena na pět podkapitol. První kapitola se věnuje horní končetině, kde je rozebrán její prenatální a postnatální vývoj, funkce horní končetiny jako celku, ale i konkrétní funkce horní končetiny v batolecím věku. Dále je zde představena kinetika a kinematika ramenního kloubu. Druhá kapitola rozebírá pojem vývojová vada a podává popis o jejich klasifikaci a etiologii. V rámci třetí kapitoly jsou sepsány zásady rehabilitační péče u vrozených vývojových vad. Tématem čtvrté kapitoly je protetická intervence u vrozených vývojových vad horních končetin. Poslední pátá kapitola poukazuje na rizika spojená s vrozenou vývojovou vadou horních končetin. V rámci metodické části jsou detailně popsány vyšetřovací postupy potřebné ke správnému nastavení terapie a případnému odhalení dalších potencionálních problémů, které by mohly ovlivnit průběh terapie. Mezi využité vyšetřovací postupy byla zařazena anamnéza, aspekční vyšetření, antropometrické a goniometrické měření, dynamické testování páteře, svalový test, základní neurologické vyšetření, vyšetření chůze, testování soběstačnosti dle Barthel indexu a vyšetření v rámci Bobath konceptu. Úvodem do speciální části je vstupní vyšetření, na jehož podkladě je sestaven krátkodobý a dlouhodobý rehabilitační plán a definovány cíle rehabilitační péče. Zbývající část této kapitoly tvoří popisy průběhů jednotlivých rehabilitačních jednotek. V kapitole výsledky jsou porovnávané hodnoty vstupního a výstupního vyšetření, s porovnáním funkčních nedostatků před započetím terapie, a následně po jejím konci. Informace jsou zpracovány formou tabulek a krátkých slovních popisů. Diskuse hodnotí dosažené výsledky a shrnuje postupy použité při zpracování práce. V rámci diskuse se hodnotí spolupráce s rodinou, období vzdoru pacientky a celková specifika práce s dětským pacientem. Dále otevírá možnosti rozšíření práce a návrhy k dosažení lepších výsledků.

The bachelor's thesis is processed as a case study of a patient with a congenital developmental defect of the upper limb classified under the diagnosis Q799, as Hemimelia of the upper limb. The bachelor's thesis describe physiotherapy intervention for a patient of toddler. The bachelor's thesis is divided into theoretical, methodological and special parts. The theoretical part is divided into five subsections. The first chapter is devoted to the upper limb, where its prenatal and postnatal development, the function of the upper limb as a whole, but also the specific function of the upper limb in toddlerhood are analyzed. The kinetics and kinematics of the shoulder joint are also discussed here. The second chapter discusses the concept of developmental disability, and provides a description of their classification and etiology. In the third chapter, the principles of rehabilitation care for congenital developmental defects are written. The topic of the fourth chapter is prosthetic intervention in congenital developmental defects of the upper limbs. The last, fifth chapter points out the risks associated with congenital developmental defects of the upper limbs. The methodological part describes in detail the investigation procedures needed to set up the therapy correctly and possibly reveal other potential problems that could affect the course of the therapy. The examination procedures used included anamnesis, aspect examination, anthropometric and goniometric measurements, dynamic testing of the spine, muscle test, basic neurological examination, gait examination, self-sufficiency testing according to the Barthel index and examination within the Bobath concept. The introduction to the special part is an initial examination, on the basis of which a short-term and long-term rehabilitation plan is drawn up and the goals of rehabilitation care are defined. The remaining part of this chapter consists of descriptions of the courses of individual rehabilitation units. In the results chapter, the values of the initial and final examination are compared, with a comparison of functional deficiencies before the start of therapy, and subsequently after its end. The information is processed in the form of tables and short verbal descriptions. The discussion evaluates the achieved results and summarizes the methods used in the processing of the work. As part of the discussion are cooperation with the family, the patient's period of resistance and the overall specifics of working with a child patient. It also opens up possibilities for expanding the work and suggestions for achieving better results.
