Předložená práce je zaměřená na možnost využití telerehabilitace při mimořádných a krizových situací. Cílem práce je objektivizovat její přínos pro rekonvalescenci v době, kdy je návštěva nemocného ve zdravotnickém zařízení nemožná nebo vysoce riziková a posoudit, zda efekt telerehabilitace je stejný jako rehabilitace kontaktní na lůžkovém oddělení či v ambulantní péči. Přínos telerehabilitace je v teoretické části disertační práce podepřen citací z odborných článků z domácí i zahraniční literatury a přednášek na vědeckých konferencích. Práce vymezuje problematiku zavádění telerehabilitace do praxe jako součást komplexní zdravotní péče. Zmíněny jsou základní požadavky na zavádění telerehabilitace do společnosti a posouzena je stávající právní legislativa telemedicíny a telerehabilitace v České republice. Na podkladě osobních zkušeností z doby nouzového stavu vynuceného pandemií Covid-19 je posouzen přínos telerehabilitace ve vybraných indikacích, které lze bez obtíží rozšířit i na další. Praktická část je založená na sledování efektu telerehabilitace u klientů po artroplastice kolenního nebo kyčelního kloubu. Pro zhodnocení výsledků sledovaných parametrů při vstupním, kontrolním a výstupním vyšetření byla zvolena kontrolní skupina klientů po stejném operačním výkonu. Výsledky efektu telerehabilitace jsou statisticky posouzeny a objektivizovány. Na základě prokázaného pozitivního přínosu telerehabilitace jsou v diskusi posouzeny možnosti využití telerehabilitace na příkladech možných antropogenních a přírodních krizových situací. Jsou zde shrnuty doporučené postupy a pravidla pro zavádění telerehabilitace do praxe na základě získaných poznatků a provedeného dotazníkového šetření mezi fyzioterapeuty a klienty. The presented work is focused on the possibility of using telerehabilitation in emergency and crisis situations. The thesis aims to objectify its contribution to recovery in a time when visiting a patient in a medical facility is impossible or high-risk and to assess whether telerehabilitation’s effect is the same as contact rehabilitation in an inpatient ward or outpatient care. The benefit of telerehabilitation in the theoretical part of the dissertation is supported by citations from professional articles from domestic and foreign literature and lectures at scientific conferences. The thesis defines the issue of implementing telerehabilitation into practice as part of comprehensive health care. The basic requirements for introducing telerehabilitation into society are mentioned as is the existing legal legislation of telemedicine and telerehabilitation in the Czech Republic. Based on personal experiences from the state of emergency imposed by the Covid-19 pandemic, the benefit of telerehabilitation in selected indications is assessed, which can be easily extended to others. The practical part is based on monitoring the effect of telerehabilitation on clients after knee or hip arthroplasty. A control group of clients after the same operation was selected to evaluate the results of the monitored parameters during the entry, control, and exit examinations. The results of the telerehabilitation effect are statistically assessed and objectified. Based on the demonstrated positive benefit of telerehabilitation, the possibilities of using telerehabilitation are assessed in the discussion using examples of possible anthropogenic and natural crises. Recommended procedures and rules for implementing telerehabilitation into practice are summarized, based on the knowledge gained and the questionnaire survey conducted among physiotherapists and clients.
