dc.description.abstract | Hluboká mozková stimulace (DBS) vnitřního pallida se u farmakorezistentních dystonických syndromů stala rutinní léčebnou metodou. Cílem disertační práce je přispět k identifikaci elektrofyziologických biomarkerů, které pomohou odlišit dystonické pacienty profitující z terapie DBS od pacientů, kterým tato metoda nepřináší žádné zlepšení. Práce se věnuje návrhu a realizaci HW a SW prostředků pro elektrofyziologické vyšetření pomocí transkraniální magnetické stimulace (TMS) a zpracování naměřených dat. Celkem bylo v navrženém protokolu vyšetřeno 22 pacientů (věk 51 ± 17 let) ve stavu zapnuté a vypnuté DBS a 22 párovaných kontrolních subjektů. V rámci sezení byla hodnocena tíže onemocnění pomocí klinických škál (BFMDS, TWSTR, UMDS) a měřena krátkolatentní intrakortikální inhibice (SICI). Data byla zpracována jak manuálně pomocí SW vytvořeného v jazyce Signal (Cambridge Electronics Design), tak i pomocí automatických algoritmů v MATLABu. Na základě statistického zpracování byly prokázány signifikantní rozdíly v hodnotách biomarkerů (latence evokovaných potenciálů, inhibiční poměr) mezi skupinou pacientů odpovídajících a neodpovídajících na DBS terapii. V závěru práce obsahuje návrh klasifikace pacientů pomocí metod strojového učení (rozhodovací strom, k-NN klasifikátor, Bayesovský klasifikátor) jako možný nástroj pro predikci efektu DBS terapie v klinické praxi. | cze |
dc.description.abstract | Deep brain stimulation (DBS) of the internal pallidum has become a routine treatment method for pharmacoresistant dystonic syndromes. The aim of the dissertation is to contribute to the identification of electrophysiological biomarkers that will help distinguish dystonic patients benefiting from DBS therapy from patients for whom this method does not bring any improvement. The work is devoted to the design and implementation of hardware and software for electrophysiological examination using transcranial magnetic stimulation (TMS) and processing of measured data. In total, 22 patients (age 51 ± 17 years) in the on and off DBS condition and 22 matched control subjects were examined in the proposed protocol. Within the session, disease severity was assessed using clinical scales (BFMDS, TWSTR, UMDS) and short-latency intracortical inhibition (SICI) was measured. The data were processed manually using custom-written scripts in the Signal language (Cambridge Electronics Design) as well as using automatic algorithms in MATLAB. Based on statistical processing, significant differences in biomarker values (latency of evoked potentials, inhibition ratio) were demonstrated between the group of patients responding and not responding to DBS therapy. At the end of the work, a proposal for classifying patients using machine learning methods (decision tree, k- NN classifier, Bayesian classifier) as a possible tool for predicting the effect of DBS therapy in clinical practice. | eng |
