Práce se zabývá minimálním zajištěním triáž pozitivního pacienta v přednemocniční neodkladné péči, která je zajišťována výjezdovou skupinou zdravotnické záchranné služby. Cílem této práce je poukázat na nedostatky v zajištění vitálních funkcí u traumaticky poraněného pacienta. Teoretická část shrnuje primární a sekundární vyšetření raněného v terénu a kompletní možnosti, jak pacienta co nejefektivněji stabilizovat. Popisuje pomůcky pro zajištění dýchacích cest či cévního řečiště, jejich indikace a kontraindikace. Dále traumatická poranění hrudníku, u nichž je nezbytná okamžitá intervence. Zabývá se podáváním intravenózních roztoků a léků. Součástí je také imobilizace dle mechanismu úrazu a konkrétních poranění. Praktická část probíhala formou prospektivního observačního výzkumu a obsahuje výsledky z průzkumného šetření, které probíhalo na odděleních urgentního příjmu a traumatologie v rámci traumatologických center v hlavním městě Praze. Součástí jsou demografické informace o věku pacientů a pozitivitě konkrétních triage kritérií pro transport do traumacentra. Dále byl sledován výskyt jednotlivých mechanismů úrazů a suspektních diagnóz. Na závěr bylo hodnoceno využití konkrétních pomůcek pro zajištění vitálních funkcí a imobilizaci pacienta. The bachelor thesis deals with the minimum care for a triage positive patient in pre-hospital emergency care, which is provided by the emergency medical services´ field teams. The aim of this work is to point out the shortcomings of the provision of vital functions of a traumatically injured patient. The theoretical part summarizes the primary and the secondary examination of the wounded in the field and a complete set of options for stabilizing the patient in the most effective way. This part also describes instruments for securing the airways or venous access, and indications and contraindications for their use. Furthermore, traumatic thoracic injuries, that require immediate intervention, are introduced. This part deals with the administration of intravenous solutions and drugs. Ways of immobilization according to the mechanism of the injury and a specific type of injury are also included. The practical part contains the results of a prospective observational study, which took place in emergency departments and traumatology of the traumatological centres in the capital city of Prague. It includes information about the age of patients and the positivity of specific triage criteria for transport to the trauma centre. The occurrence of individual mechanisms of injuries and suspicious diagnoses were monitored. Finally, we the use of specific tools for ensuring vital functions and for immobilizing the patient were recorded.
