Fyzioterapie u pacientů se srdeční podporou HeartMate

Abstract

Předmětem bakalářské práce je vliv fyzioterapie u pacientů s dlouhodobou mechanickou srdeční podporou HeartMate 3, která se využívá v terapii terminálního srdečního selhání. Práce je zpracována formou kazuistiky a je rozdělena na část teoretickou a speciální. Teoretická část se zabývá přehledem současného stavu, přičemž shrnuje anatomii a fyziologii kardiovaskulárního systému, přináší poznatky o chorobě srdečního selhání, základní informace o mechanických srdečních podporách se zaměřením na zařízení HeartMate 3 a popisuje možnosti rehabilitační péče u kardiochirurgických zákroků. V kapitole Metodika je uveden metodický postup, dále aplikované vyšetřovací a terapeutické metody, které byly využity v praktické části práce. Speciální část je věnována vstupním datům pacienta včetně anamnézy, výpisu ze zdravotní dokumentace a indikace k rehabilitaci. Dále zahrnuje vstupní kineziologický rozbor, krátkodobý a dlouhodobý fyzioterapeutický plán, a podrobně popisuje průběh jednotlivých rehabilitačních jednotek. Závěr speciální části obsahuje výstupní kineziologický rozbor a zhodnocení přínosu terapie. Výsledky ukazují pozitivní vliv fyzioterapie na pooperační stav pacienta, zejména v oblasti svalové síly, dynamiky hrudníku, dechového stereotypu a chůzi. Kapitola Diskuze pojednává o problematice dlouhodobých mechanických srdečních podpor, možných pooperačních komplikacích a jejich souvislosti s průběhem a výsledky rehabilitační péče v interakci s odbornými studiemi.

The subject of the bachelor thesis is the effect of physiotherapy on patients with long-term mechanical heart support HeartMate 3, which is used as the treatment for terminal heart failure. The thesis is processed in the form of patient case study and is divided into theoretical and special part. The theoretical part includes an overview of the current state, summarizing the anatomy and physiology of the cardiovascular system, provides knowledge about heart failure, basic information about mechanical heart support, focusing on the HeartMate 3 device, and describes the options of rehabilitation care after cardiac surgery. The Methodology chapter presents the methodological strategy, further applied examination and therapeutic methods, which were used in the practical part of the thesis. The special part is dedicated to the patient's input data, anamnesis, medical records and indication for rehabilitation. It also includes an initial kinesiological analysis, short-term and long-term physiotherapy plan, and describes in detail the course of individual rehabilitation units. The conclusion of the special part contains the final kinesiological analysis and the evaluation of the benefits of the therapy. The results show a positive effect of physiotherapy on the patient's postoperative condition, especially in the areas of muscle strength, chest dynamics, respiratory stereotype and walking. The Discussion chapter deals with the issue of long-term mechanical heart support, possible postoperative complications and their connection with the course and results of rehabilitation care in interaction with professional studies.
