Specifika ošetřovatelské péče u pacientů s epilepsií

Abstract

Bakalářská práce se zabývá problematikou ošetřovatelské péče u pacientů s epilepsií. V teoretické části je uvedena základní charakteristika tohoto onemocnění, popsány příčiny, patogeneze, klinické příznaky, diagnostika, první pomoc, léčebná a ošetřovatelská péče. Tato část vychází z analýzy odborné literatury. Praktická část práce je zpracována formou čtyř případových studií, které byly získány na jednotce intenzivní péče Neurochirurgické kliniky dětí a dospělých Fakultní nemocnice Motol a 2. lékařské fakulty Univerzity Karlovy. Případové studie byly cíleně zpracovány u pacientů, kteří trpí farmakorezistentní epilepsií a z toho důvodu jim byla indikována epileptoresekční operace. U dvou z těchto pacientů byla operace provedena v celkové anestezii, u dvou dalších byla operace provedena s využitím awake metody, která spočívá v přivedení pacienta k plnému vědomí během anestezie. Na základě ošetřovatelského procesu byly u těchto pacientů stanoveny ošetřovatelské diagnózy podle Taxonomie North American for Nursing Diagnosis Assotiation, NANDA I., z nichž byly následně řešeny diagnózy specifické pro tento perioperační průběh. Cílem práce bylo podchytit specifika ošetřovatelské péče u konkrétních pacientů, u nichž se vyskytla epilepsie ve farmakorezistentní formě a bylo nutné ji řešit operačně a vytvořit na podkladě získaných informací doporučení k ošetřovatelské péči o tyto pacienty, které by doplnilo běžně aplikované postupy. Analýzou případových studií bylo zjištěno, že mezi specifika ošetřovatelské péče, která je nutno řešit, patří mj. strach z výkonu a jeho následků, zvýšené riziko infekce, výrazné riziko pádu, akutní pooperační bolest, riziko omezené komunikace, riziko poškození paměti a s výše uvedenými problémy spojený nutný stálý dohled a zajištění bezpečnosti pacienta, dále potřeba dodržování rituálů, na které byl pacient zvyklý (podávání léků, stravy, hygiena), ale i zajištění periferního žilního katetru pro riziko opakovaných záchvatů a nutnost jejich farmakologického zajištění, nutnost dostatečné informovanosti nelékařského zdravotnického personálu zejména o průběhu záchvatů u konkrétního pacienta. V neposlední řadě je nutné i zajištění správné polohy pacienta, tzn. lůžko ve 35–40 stupních po resekční operaci a zvednutí postranic ke zvýšení bezpečí.

This bachelor's thesis deals with the issue of nursing care for patients with epilepsy. The theoretical part presents the basic characteristics of this disease, describes causes, pathogenesis, clinical signs, diagnostics, first aid, therapeutic and nursing care. This section is based on an analysis of the scientific literature. Practical part of the thesis is formed by four case studies, which were obtained in the intensive care unit of the Neurosurgical Clinic of Children and Adults of the Motol University Hospital and the 2nd Medical Faculty of Charles University. Case studies have been targeted on patients, who suffer from pharmacocorrosion-resistant epilepsy and have therefore been indicated with epileptoresectsurgery. In case of first two patients, the operation was performed under general anaesthesia, in case of two others the operation was performed using awake method, which involves bringing the patient to full consciousness during anaesthesia. Based on the nursing process, these patients were given nursing diagnoses by the Taxonomy of North American for Nursing Diagnosis Assotiation, NANDA I., which were later used to address diagnoses specific to this perioperative course. Aim of this work was to capture the specificities of nursing care on specific patients, who experienced epilepsy in a pharmaco-resistant form and it was necessary to address it operationally and to develop recommendations for nursing care for these patients based on the information gathered, to complement routinely applied procedures. An analysis of the case studies found, that the specifics of nursing care that need to be addressed include, inter alia, fear of performance and its consequences, increased risk of infection, significant risk of fall, acute post- operative pain, risk of limited communication, risk of memory impairment and the problems mentioned above, the need for constant supervision and safety of the patient, the need for adherence to the rituals to which the patient was accustomed, (medication, diet, hygiene) but also the provision of a peripheral venous catheter for the risk of recurrent seizures, and the need for their pharmacologic, the need for adequate awareness of non-medical staff in particular, about the course of seizures in a particular. Last but not least, it is necessary to ensure the correct position of the patient, i.e. a bed in 35-40 degrees after resection surgery and raising the sides to increase safety.
