Hodnocení funkčního stavu a kvality života pacientů po artroplastice kolenního kloubu

Abstract

Hodnocení funkčního stavu a kvality života pacientů po artroplastice kolenního kloubu Operace artroplastika kolenního kloubu znamená výraznou změnu pro pacienty jak po fyzické, tak po psychické stránce. Tato diplomová práce řeší kvalitu života a funkční stav pacientů po artroplastice kolenního kloubu. Cílem je zhodnotit kvalitu života a funkční stav pacientů, posoudit korelaci funkční samostatnosti a kvality života a dále zhodnotit citlivost jednotlivých dotazníků. Vyhodnocení probíhalo na základě dotazníkového šetření pomocí generického nástroje EQ-5D-3L a specifického nástroje Osteoarthritis Knee and Hip Quality of Life. Funkční stav byl hodnocen pomocí nástroje Functional independence meassure. Na základě výsledků bylo zjištěno, že mezi faktory ovlivňující spokojenost pacientů s kvalitou života a funkčním stavem po zákroku patří zejména bolest a psychický stav jedince, dále pak očekávání pacientů před zákrokem. Byla potvrzena korelace mezi funkční soběstačností a kvalitou života. Nástroje pro měření kvality života jsou považovány oba za citlivé vůči změnám v kvalitě života. Na základě zjištěných výsledků lze konstatovat, že došlo ke zlepšení jak v kvalitě života pacientů po zákroku, tak ke zlepšení funkčního stavu pacientů. Mezi hlavní faktory, které ovlivňují spokojenost pacientů patří bolest a psychický stav, což se shoduje také s výsledky analyzovaných studií. Pilotní studie probíhala během pandemie Covid-19, což lze považovat za jeden z faktorů, který měl vliv na počet probandů zařazených do studie a na získané výsledky. Stanovené cíle lze považovat za splněné. V rámci dalšího směřování práce je vhodné se věnovat detailní analýze očekávání pacientů před zákrokem a pozitivnímu ovlivnění rizikových faktorů zjištěných v této práci.

Evaluation of functional status and quality of life of patients after kneearthroplasty Knee arthroplasty surgery means a significant change for patients both physically and mentally. This diploma thesis deals with the evaluation of the quality of life and functional independence of patients after knee arthroplasty. The aim is to evaluate the quality of life and functional status of patients, assess the correlation between functional independence and quality of life and further evaluate the sensitivity of individual questionnaires. The evaluation was based on a questionnaire survey using the generic questionnaire EQ-5D-3L and the specific questionnaire Osteoarthritis Knee and Hip Quality of Life. Functional status was assessed using the Functional Independence Measurement method. Based on the results, it was found that among the factors influencing patients' satisfaction with the quality of life and functional status after the operation are mainly the pain and mental state of the individual, as well as the expectations of patients before the operation. The correlation between functional self-sufficiency and quality of life was confirmed. Quality of life measurement tools are both considered sensitive to changes in quality of life. Based on the results, it can be stated that there has been an improvement in both the quality of life of patients after the operation and an improvement in the functional status of patients. The main factors that affect patient satisfaction include pain and mental state, which also agrees with the results of the analyzed studies. The pilot study took place during the Covid-19 pandemic, which can be considered as one of the factors that influenced the number of probands included in the study and the results obtained. The set goals can be considered fulfilled. As part of the further direction of the work, it is appropriate to pay a detailed analysis of patients' expectations before the operation and the positive influence of risk factors identified in this work.
