Rizika poskytování první pomoci dětem vyžadujícím zvláštní péči

Abstract

Bakalářská práce se zabývá možnými riziky vzniklými při poskytování první pomoci dětem vyžadujícím zvláštní péči. Jedná se o problematiku, která je v současné době velmi opomíjena a zdravotničtí záchranáři neznají správné postupy a pravidla při komunikaci s osobou s určitým postižením. Výjezdy posádek ZZS k těmto osobám nejsou sice časté, nicméně by měly být očekávané. Teoretická část je zaměřena na děti se zdravotním postižením. V první části jsou uvedeny základní etické náležitosti související s dětským pacientem a s tím navazující jejich práva. Druhá část informuje o publikacích, podle kterých se klasifikují jednotlivé diagnózy a určuje stupeň postižení. Další část definuje komunikaci s dětmi všeobecně, podkapitola je pak již věnovaná odlišnostem komunikace při kontaktu s postiženými dětmi. Zbytek teoretické části se zabývá jednotlivými skupinami dětí s postižením, klasifikací daných postižení, správnou komunikací a kompenzačními pomůckami při komunikaci s nimi. Praktická část obsahuje čtyři kazuistiky dětí s různým sluchovým postižením. Na konci každé kazuistiky se nachází vyhodnocení daného případu. Tato část je zakončena porovnáním jednotlivých kazuistik mezi sebou a poukazuje na odlišnosti sluchového postižení u každého jedince. Součástí je odborná praktická přednáška první pomoci na škole pro neslyšící. Dětem byly ukázány postupy první pomoci při KPR, zástavě krvácení, popáleninách a zlomeninách. Výsledky znalostí dětí před a po přednášce jsou prezentovány prostřednictvím grafů. Pro doplnění informací jsou formou polostandardizovaných rozhovorů zhodnoceny zkušenosti zdravotnických pracovníků po setkání s dětmi se sluchovým postižením. V závěru jsou navržena opatření k eliminaci rizik poskytování první pomoci dětem vyžadujícím zvláštní péči. V diskuzi se pojednává o komunitě Neslyšících a jejich problému začlenění se do světa slyšících, tedy o problému získání rovnocenného přístupu. Shrnuje jejich cestu k cíli v průběhu let a hodnotí současný stav populace neslyšících. Významný je jejich způsob komunikace, a především jejich možnosti přivolání ZZS formou speciálních čísel a aplikací. V závěru práce se vyhodnocuje splnění cíle bakalářské práce. Tímto cílem bylo zhodnotit rizika poskytování zdravotnické péče a následně najít opatření k eliminaci těchto rizik. Dále je zde uveden i přínos bakalářské práce.

The bachelor thesis is about the possible risks caused by providing first aid to children requiring special care. This is a problem that is currently neglected and paramedics do not know the correct process and rules of communication with someone with a particular disability. Ambulance services to these people are not frequent, however, they should be expected. The theoretical part is focused on children with disabilities. In the first part are presented the basic ethical requirements and rights related to the child patient. The second part informs about publications according to classification of individual diagnoses and determines the degree of disability. Next part defines general communication with children, the subchapter is dedicated to communication differences with disabled children. The rest of the theoretical part deals with individual groups of children with disabilities, classification of disabilities, correct communication and compensatory aids in communication with them. The practical part contains four case history of children with different hearing loss. At the end of each case, there is an evaluation of that case. This section ends with a comparison of each case between each other and points out the differences of the hearing disability of each individual. This part also contains a practical lecture of first aid at a school for deaf children. Children were shown first aid procedures for CPR, heavy bleeding treatment, burns and bone fractures. The results of the children's knowledge before and after the lecture are presented via graphs. To supplement the information, there are semi-standardized interviews with healthcare workers about their experiences after meeting the hearing impaired children. In the end, measures are proposed to eliminate the risks of providing first aid to children requiring special care. The discussion is about the Deaf community and their problem of joining the world of hearing, which means the problem of getting an equal approach. It summarizes their path to their goal through the years and evaluates the current state of the deaf population. Significant is their way of communication and, above all, their ability to call emergency medical services thanks to special numbers and applications. In closing of the thesis is evaluated the aim of the bachelor thesis. This aim was to assess the risks of health care provision and subsequently to find elimination measures of these risks.
