Problematika ošetření ran dekubitů v intenzivní ošetřovatelské péči - nové trendy

Abstract

Ústředním tématem této bakalářská práce je problematika ošetřovatelské péče u pacientů, kterým se vznikly rány typu dekubitů během hospitalizace na odděleních intenzivní péče. Léčba dekubitů je stále aktuálním tématem nejen na odděleních intenzivní péče, ale i na dalších odděleních, zejména u dlouhodobě hospitalizovaných pacientů, protože počet těchto pacientů stále stoupá. Hlavním cílem práce je zhodnocení této péče a prevence vzniku dekubitů u zvolených pacientů. Na základě získaných dat je plánováno sestavení optimálního postupu prevence. Práce je oddělena na dvě základní části, tj. na část teoretickou a část praktickou. V teoretické části práce jsou charakterizovány nejdůležitější pojmy, jako je stavba kůže, rána a hojení ran. Dále teoretická část detailně propracovává problematiku dekubitů, a to zejména jejich klasifikaci, prevenci, místa vzniku, faktory ovlivňující dekubity, jejich ošetřování, používané metody léčby včetně přípravků a nejnovější poznatky ohledně léčby dekubitů. Praktická část formou kvalitativního výzkumu, je založena na sestavených kazuistikách, ve kterých jsou zahrnuta nejdůležitější data z dokumentace pacientů. Zaměřuje se především na anamnézu, průběh hospitalizace a ošetřovatelskou dokumentaci týkající se ošetřování ran a dekubitů.

This bachelor thesis explores various issues related to the nursing care for patients who have developed decubitus ulcers during their stay in intensive care unit. Treatment of decubitus ulcer is of a significant importance not only in the intensive care unit but also in other departments, especially in the case of long-term hospitalized patients. Moreover, number of hospitalized and lying patients is still increasing. The main goal of this work is to evaluate the care and the prevention of decubitus ulcers for number of selected patients and present an optimal prevention practice based on the gathered data. The thesis is divided into two basic parts, the theoretical part and the practical part. In the theoretical part of this thesis, the most important related terms such as skin construction, wound and wound healing are explained. Further, the theoretical part deals with the details of the problem of decubitus, especially the classification, prevention, origin, factors influencing the decubitus, and treatment, including healing preparations and the latest knowledge about treatment of decubitus. The empirical part is mainly based on assembled case reports, which include crucial data from patient documentation. It focuses primarily on the history, course of hospitalization and nursing documentation on wound healing and treatment of decubitus ulcers.
