Problematika bolesti a možností jejího ovlivnění v intenzivní ošetřovatelské péči

Abstract

Tématem bakalářské práce je problematika pooperační bolesti na odděleních intenzivní péče. Při práci na intenzivních odděleních i v rámci zdravotnické záchranné služby se zdravotnický záchranář s pacienty trpícími bolestmi každodenně setkává, jedná tedy o nezbytnou součást jeho práce. Zdravotnický záchranář si musí být jistý v komunikaci s takovými pacienty, být empatický a v rámci svých kompetencí umět bolest tišit. Musí mít na paměti, že vnímání bolesti je subjektivní záležitostí a každý pacient může reagovat na bolest jiným způsobem. Bolest může rovněž hrát významnou roli při diagnostice. V teoretické části své bakalářské práce jsem nastínila základní informace týkající se bolesti. Vymezila jsem pojmy jako akutní a chronická bolest, hodnocení a sledování bolesti a možnosti jejího ovlivnění. Závěrečnou, metodickou část práce jsem věnovala popisu kazuistik u 4 pacientek po ortopedické operaci (totální endoprotéza kolenního kloubu). V rámci své praxe jsem sbírala data týkající se pooperační bolesti pacientek, zapisovala, jak bolest subjektivně hodnotily, a podílela se na analgetické léčbě. Získané informace jsem zpracovala formou tabulek, kde je zobrazena intenzita bolesti dle hodnocení pacientek a následná analgetická léčba.

This thesis focuses on the issue of post-surgical pain in intensive care departments. In intensive care and emergency medical services, a paramedic meets patients who suffer from pain every day. Therefore dealing with pain is an essential part of his/her work. A paramedic must be confident in communicating with such patients, empathic and able to ease the pain within his/her competencies. He/she must keep in mind that pain perception is a subjective matter and that each patient can respond to pain in a different way. Pain can also play a significant role during diagnosis. In the theoretical part of my bachelor thesis I outlined the basic information regarding pain. I defined the concepts of acute and chronic pain, the evaluation and monitoring of pain and the possibility of influencing them. I devoted the final, methodical part, to a description of case studies of four patients after orthopedic surgery (total knee joint endoprotease). During my internship, I collected data on post-surgical pain of the patients, recorded how they subjectively evaluated their pain, and participated in analgesic treatment. I processed the received information and created tables showing pain intensity according to patient assessment and subsequent analgesic treatment.
