Bakalářská práce pojednává o problematice vzácných nádorů nadledvinek (paragangliomu a feochromocytomu) a zaměřuje se na genovou expresi programmed cell death 1 (dále jen PD-1), které využívají nádorové buňky k potlačení imunitní odpovědi. PD-1 by mohla být terapeutickým cílem podobně jako u jiných nádorů. Obecná část se zaměřuje na neuroendokrinní nádory, změny metabolismu katecholaminů, protinádorovou imunitu, molekuly PD-1 a její ligandy PD-ligand 1/2 (PD-L1/2, programmed death domain ligand 1/2), funkci inhibiční dráhy PD-1/PD-L1 a nádorovou toleranci. Následně jsou popsány dostupné diagnostické a terapeutické postupy. Praktická část se věnuje porovnání genové exprese PD-1 ve vzorcích tkáně onkologických pacientů s paragangliomem či feochromocytomem se vzorky tkáně zdravého jedince. Ke zjištění hodnot genové exprese PD-1 byly použity metody izolace ribonukleové kyseliny, reverzní transkripce z ribonukleové kyseliny na komplementární deoxyribonukleovou kyselinu a kvantitativní polymerázové řetězové reakce v reálném čase. Z výsledků je patrná odlišnost genové exprese PD-1 mezi skupinou nemocných pacientů a zdravých jedinců, z čehož vyplývá, že je vyšší exprese PD-1 v nádorové tkáni. Exprese PD-1 je ovlivňována mnoha imunitními a genetickými mechanismy. Výsledky by mohly přispět k porozumění role imunoregulačních molekul u nádorového onemocnění a následné optimalizaci léčby. This bachelor's thesis is focusing on topic of rare tumours of adrenal glands, (paraganglioma and pheochromocytoma) and targeting gene expression of the programmed cell death 1 (PD-1) which is used by tumour cells to inhibit the immune response. PD-1 could be a therapeutic target similarly as in other types of tumours. The theoretical part describes neuroendocrine tumours, changes in catecholamine metabolism, antitumor immunity, PD-1 molecule and its ligands PD-ligand 1/2 (PD-L1 and PD-L2), the function of PD-1/PD-L1 inhibitory pathway and tumour tolerance. Consequently, diagnostic and therapeutic approaches are described. The practical part aim at comparison of the gene expression of PD-1 in samples of tissue of oncological patients affected by paraganglioma or pheochromocytoma and samples of the healthy individuals. To ascertain gene expression of PD-1, methods of the ribonucleic acid isolation, reverse transcription to the complementary deoxyribonucleic acid and quantitative real-time polymerase chain reaction were used. The results reveal that there is a noticeable difference in the PD-1 gene expression of the tissue of affected patients and healthy people. On the basis of this results, it can be concluded that there increased expression of PD-1 in the tumour tissue. The PD-1 expression is influenced by many immune and genetic mechanisms. The results could contribute to a better understanding of role of immunoregulatory molecules in tumour and eventual optimization of therapy.
