Telerehabilitace jako vhodný doplněk ambulantní péče periferní parézy nervus facialis

Abstract

Bakalářská práce se zaměřuje na využití telerehabilitace u obrny lícního nervu. Zabývá se tím, zda je telerehabilitace v domácím prostředí stejně účinná jako klasická rehabilitace a zda se dá využít u všech pacientů s tímto onemocněním. Bakalářská práce také shrnuje problematiku obrny lícního nervu a její léčbu. Tato práce je rozdělena do dílčích kapitol. V první kapitole je shrnutí teoretických poznatků tohoto onemocnění a představení telemedicíny s jejími odvětvími, do nichž patří i zmíněná telerehabilitace. V další kapitole jsou popsány vyšetřovací metody, metody použité v terapeutických jednotkách a doporučené další vhodné metody pro toto onemocnění. Ve speciální části jsou tři kazuistiky pacientek s obrnou lícního nervu. První pacientka trpící pooperační periferní parézou těžkou dysfunkcí (dle House-Brackmannovy škály), druhá pacientka s mírnou dysfunkcí a třetí pacientka s dysfunkcí střední. U pacientek byla odebrána anamnéza se vstupním vyšetřením a určen krátkodobý a dlouhodobý rehabilitační plán. Ve výsledcích je porovnání vstupního a výstupního kineziologického rozboru a zhodnocení výsledků terapie. U pacientky s těžkou dysfunkcí nemohla být telerehabilitace provedena z důvodu nedosažení potřebné svalové síly, u druhé pacientky se podařilo rehabilitovat prostřednictví Skype. U třetí pacientky také, ale vzhledem k jejímu vyššímu věku jsem hodnotila léčbu jako neefektivní.

Bachelor thesis focuses on the use telerehabilitation of facial nerve palsy. It deals with home telerehabilitation is as effective as classical rehabilitation and whether it can be used in all patients with this disease. The bachelor thesis also summarizes the issue of facial nerve palsy and treatment. This thesis is divided into sub-chapters. In the first chapter is a summary of the theoretical knowledge of this disease and the introduction of telemedicine with its branches, including the mentioned telerehabilitation. The next chapter describes the methods of investigation, methods used in therapeutic units and recommended other appropriate methods for this disease. In a special part are three case reports of patients with facial nerve palsy. First patient suffering from postoperative peripheral paresis with severe dysfunction (according to House-Brackmann's scale), second patient with mild dysfunction and third patient with moderate dysfunction. Patients were taken anamnesis and determined a short-term and long-term rehabilitation plan. The results are a comparison of the input and output kinesiological analysis and the evaluation of the results of the therapy. In a patient with severe dysfunction, telerehabilitation could not be performed due to lack of necessary muscle strength, the other patient managed to rehabilitate through Skype. The third patient also, but because of her senior age, I considered it ineffective.
