Vliv časné fáze fyzioterapie u pacientů s totální endoprotézou kolenního kloubu

Abstract

Tato bakalářská práce se zabývá určením vlivu časné fyzioterapie u pacientů s totální endoprotézou kolenního kloubu. Cílem je objektivně zhodnotit a určit vliv časně započaté intenzivní fyzioterapie porovnáním skupiny probandů, kteří nastoupili k fyzioterapii přímým překladem z lůžka ortopedického oddělení a druhou skupinou probandů, kteří nastupovali k intenzivní fyzioterapii v Rehabilitačním centru Máj, Lázně Poděbrady, a.s. až po více než 2 měsících po operaci. V teoretické části jsou popsány anatomické, kineziologické a biomechanické poznatky. Tato část se také zabývá problematikou osteoartrózy a totálních endoprotéz, popisuje diagnostiku artrózy, indikace a kontraindikace operačního řešení a typy kloubních náhrad. Závěrem teoretické části je popsán návrh časné fáze fyzioterapie po dobu hospitalizace. Speciální část popisuje vyšetřovací a terapeutické postupy, které byly využity v terapii probandů. Ve speciální části je dále zpracováno deset kazuistik ze dvou skupin probandů, které obsahují anamnestické údaje, vstupní kineziologický rozbor, týdenní přehled absolvovaných procedur v RC Máj, krátkodobý a dlouhodobý fyzioterapeutický plán. Dosažené výsledky dle výstupního a kontrolního kineziologického rozboru jsou zpracovány v tabulkách a vyhodnoceny u každého probanda. V diskuzi jsou porovnány obě skupiny probandů a jejich stavu před a po fyzioterapeutické péči a s odstupem 2 měsíců od ukončení fyzioterapie a tím je zhodnocen vliv časné fáze fyzioterapie u této diagnózy a porovnání s prostudovanými zdroji.

This bachelor thesis deals with determination of the influence of early phase of physiotherapy for patients with total knee joint replacements. Its aim is to objectively evaluate and determine the influence of early initiated intensive physiotherapy by comparing a group of probands who began physiotherapy by direct transfer from an orthopaedic ward bed with a second group of probands who were taken for an intensive physiotherapy in Rehabilitační centrum Máj, Lázně Poděbrady, a.s. after more than 2 months after the surgery. The theoretical part of this bachelor thesis describes anatomical, kinesiological and biomechanical knowledge. This part also deals with the issues covering osteoarthritis and total joint replacements, and describes diagnosis of arthrosis, indications and contraindications of surgical treatment and types of joint replacements. The conclusion of the theoretical part includes description of the proposed early phase of physiotherapy during hospitalization. The special part of this bachelor thesis describes the investigation and therapeutic procedures to be used in probands therapy. Further, the special part contains ten case histories from two groups of probands that include medical history data, input kinesiological analysis, weekly summary of undertaken spa treatments in RC Máj, and short-term and long-term physiotherapeutic plan. The results achieved according to the output and control kinesiological analysis have been processed in the form of tables and evaluated for each proband. Both groups of probands and their conditions before and after the physiotherapeutic care and conditions two months after the end of physiotherapy have been compared in the form of a discussion, thus evaluating the effect of the early phase of physiotherapy on this diagnosis including a comparison with the sources studied.
