Fyzioterapie u pacientů s impingement syndromem

Abstract

Tato bakalářská práce je zaměřena na problematiku fyzioterapeutických postupů u diagnózy impingement syndrom. Bakalářská práce je zpracována formou tří kazuistik u uvedené diagnózy. Cílem obecné části této práce je podat ucelený pohled na průběh a možnosti léčby tohoto onemocnění. Bolestivá onemocnění ramenních pletenců a ramenních kloubů jsou u pacientů na ambulantních rehabilitačních odděleních velmi častá a subakromiální impingement syndrom patří právě mezi nejčastější poruchy ramene. Jedná se o onemocnění, při kterém jsou bolestivě utlačeny měkké tkáně v subakromiálním prostoru ohraničeným lopatkou, korakoakromiálním vazem a hlavicí kosti pažní. K útlaku měkkých tkání dochází zejména při upažení. Součástí teoretické části jsou základní anatomické a kineziologické poznatky týkající se pletence ramenního a ramenního kloubu, definice impingement syndromu, jeho stádií a diagnostické a terapeutické postupy. Tyto postupy jsou rozděleny na základní konzervativní léčbu ve formě rehabilitace, farmakoterapie a fyzikální terapie a v případě neúspěšné konzervativní terapie následuje operativní řešení. Dále jsou zde popsány vyšetřovací a terapeutické postupy, použité ve speciální části. Ve speciální části jsou zpracovány tři kazuistiky. U každé z nich byl nejprve vypracován vstupní kineziologický rozbor a na jeho základě byl stanoven krátkodobý a dlouhodobý rehabilitační plán. Poté jsou popsány jednotlivé individuální terapeutické jednotky, a na závěr je vždy proveden výstupní kineziologický rozbor. Dále jsou porovnány výsledky vstupního a výstupního vyšetření, které prokázaly, že zvolené fyzioterapeutické postupy byly vhodné a účinné. Je dosaženo očekávaných výsledků a terapeutické jednotky lze uznat za efektivní. Závěrem bakalářské práce je diskuze o dané problematice.

This bachelor thesis deals with physiotherapy methods for those who were diagnosed shoulder impingement syndrome. In the thesis, we can find three case interpretations of the diagnosis. The aim of the general part is to provide a wholesome perspective on the progress and treatment of this condition. The patients´painful problems of shoulder girdles and shoulder joints are very common in outpatient departments and subacromial impingement syndrome is one of the most frequent issues. It is a disease caused by impingement the soft tissues in the sub-acromial space between shoulder blade, coracoclavicular ligament and head of the humerus. The soft tissues are affected especially while stretching the arms out sideways. The theoretic part includes the basic anatomical and kinesiological findings concerning shoulder girdle and shoulder joint, the definition of impingement syndrome, also its stages and the diagnostic and therapeutic methods. Those techniques are divided into a basic conservative treatments such as rehabilitation, pharmacological treatment and physical therapy followed by surgery in case of unsuccess. As well as it describes the examination and therapeutic methods used in specialize part of the work. There are three case interpretations in the special part of the bachelor thesis. Each case interpretation consists of an input kinesiological analysis and then it was followed by a short-term and a long-term rehabilitation plans. On the top of that, the particular individual therapeutic units are described in the bachelor thesis as well. In the end, an output kinesiological analysis is done. Further the results of the input and output examination are compared to prove that the particular physiotherapeutic methods were appropriate and successful. This thesis reached the expected results and the therapeutic unit can be considered as effective. The bachelor thesis is ending in a discussion about the topic.
