Vliv respirační fyzioterapie u dětských kardiologických pacientů

Abstract

Bakalářská práce se zabývá zařazením respirační fyzioterapie do ucelené rehabilitační péče u dětského pacienta s vrozenou vývojovou vadou srdce. Práce je rozdělena do tří hlavních částí: současný stav, metodika a speciální část. Kapitola současný stav shrnuje základní poznatky vývoje, morfologie a fyziologie dýchacího a kardiovaskulárního systému, popisuje klasifikaci vrozených srdečních vad, metodické postupy respirační fyzioterapie a hygienu dýchacích cest v pediatrii. Kapitola metodika uvádí všechny použité vyšetřovací i terapeutické metody, které byly využity ve speciální části. Dále zahrnuje popis pracoviště, ve kterém probíhaly všechny terapeutické jednotky. Speciální část se zabývá problematikou dětského pacienta s vrozenou srdeční vadou a vlivem respirační fyzioterapie u těchto vrozených vývojových vad. Práce je zpracována formou kazuistiky a je zde uvedena anamnéza, výpis ze zdravotnické dokumentace a vypracování vstupního a výstupního kineziologického rozboru. Na základě výsledků těchto vyšetření byl sestaven krátkodobý a dlouhodobý rehabilitační plán. Jsou zde popsány jednotlivé individuální terapeutické jednotky, zhodnocení dosažených výsledků a efektu terapie. Diskuse se zabývá specifickým přístupem k dítěti s vrozenou srdeční vadou a je soustředěna na posouzení účinků respirační fyzioterapie. Dále uvádí hodnoty saturace a BMI indexu a zdůrazňuje problematiku oběhové nestability pacientky s nutností budoucí re-operace po dosažení doporučené hmotnosti pacientky, které není schopna dosáhnout.

This bachelor thesis focuses on the inclusion of respiratory physiotherapy into a comprehensive rehabilitation program of a child patient with a congenital heart defect. The chapter on the current state summarizes the basic findings on the development, morphology and physiology of the respiratory and cardiovascular system. It then continues to describe the classification of congenital heart defects, methodological procedures of respiratory physiotherapy and the respiratory hygiene in pediatrics. The methodology chapter lists all examination and therapeutic methods used in the special part. It also includes a description of the workplace where all therapeutic units were conducted. The special part describes the topic of treating a child patient with a congenital heart defect and the effect of respiratory physiotherapy on such congenital disorders. The thesis is prepared in the form of a case study stating the anamnesis, medical record and the initial and discharge kinesiological analysis. Based on the results a short- and long-term rehabilitation plan was devised. It includes a description of individual therapeutic units and an evaluation of achieved results and effects of the therapy. The discussion describes the specific approach when treating a child patient with a congenital heart defect and concentrates on assessing the effects of respiratory physiotherapy. It further lists the saturation and BMI index values and emphasizes the issue of circulatory instability of the patient with the need of future surgery upon reaching the recommended weight of the patient, which she is unable to obtain.
