Tato disertační práce se věnovala problematice korekce astigmatismu torickou intraokulární čočkou (IOL) u pacientů se senilní kataraktou. Cílem práce bylo zavedení nové metodiky tvorby operačního plánu s cílem snížení residuálního astigmatismu oproti dosavadní metodice, analýza pooperačních zrakových funkcí a v neposlední řadě aproximace vlivu polohových faktorů torické IOL na výsledný residuální astigmatismus. Pooperační hodnocení zrakových funkcí 90 očí operovaných dle nově navržené metodiky prokázalo snížení residuálního refrakčního astigmatismus na průměrnou hodnotu 0,38 ± 0,35 D (z původních 0,61 ± 0,37 D), přičemž 84,44 % očí nemělo hodnoty residua vyšší než 0,50 D. Průměrná aritmetická chyba predikce residua byla rovna 0,40 ± 0,34 D. Realizována byla také pooperační vektorová analýza, kdy indukovaný rohovkový astigmatismus byl roven 0,32 D v ose 15° pro pravé a 0,30 D v ose 3° pro levé oči. Pro pooperační analýzu a objektivizaci faktorů polohy torické IOL se podařilo úspěšně vytvořit originální software. Po grafické analýze pooperačních snímků byla průměrná decentrace IOL vůči visuální ose v rovině vertikální rovna 0,20 ± 0,19 mm a v rovině horizontální 0,26 ± 0,19 mm, průměrný nejvyšší náklon = 4,58 ± 2,09° (medián = 1,00°) a průměrná rotace vůči plánované úhlové poloze 4,13 ± 3,33° (medián = 3,00°). Dále byla popsána metodika aproximace vlivu jednotlivých polohových faktorů IOL na residuální refrakční astigmatismus, kdy průměrný vektorový rozdíl odhadu od reálného pooperačního výsledku byl roven 0,51 ± 0,33 D. Hodnotu mediánu rozdílu = 0,50 D odhadu nepřekročilo 68,89 % očí. Výstupem práce je originální návrh komplexního řešení a hodnocení pooperačních výsledků intraokulární korekce astigmatismu umožňující zároveň identifikaci potenciální příčiny refrakčního překvapení v oftalmologické praxi. This dissertation focused on the correction of astigmatism with toric intraocular lens (IOL) in patients with senile cataract. The aim of the thesis was to introduce a new methodology of surgical plan design in order to reduce the residual refractive astigmatism (RRA) compared to the existing methodology, to analyze the postoperative visual function and to approximate the impact of positional factors of toric IOL on the resulting RRA. The postoperative visual function assessment of 90 eyes operated according to the newly proposed methodology showed a reduction of residual refractive astigmatism to an average value of 0.38 ± 0.35 D (from the previous 0.61 ± 0.37 D), while 84.44 % of eyes had residual astigmatism values no higher than 0.50 D. The mean arithmetic error of the residual prediction was equal to 0.40 ± 0.34 D. Postoperative vector analysis was also performed, with induced corneal astigmatism equal to 0.32 D in the 15° axis for the right and 0.30 D in the 3° axis for the left eyes. Original software was successfully developed for postoperative analysis and objectification of toric IOL position factors. After graphical analysis of the postoperative images, the mean decentration of the IOL relative to the visual axis in the vertical plane was 0.20 ± 0.19 mm and in the horizontal plane was 0.26 ± 0.19 mm, the mean maximum tilt = 4.58 ± 2.09° (median = 1.00°), and the mean rotation relative to the planned angular position was 4.13 ± 3.33° (median = 3.00°). Furthermore, the methodology for approximating the effect of the different IOL positional factors on residual refractive astigmatism was described, with the mean vector difference of the estimate from the actual postoperative result being equal to 0.51 ± 0.33 D. The median difference = 0.50 D of the estimate was not exceeded by 68.89% of the eyes. The outcome of the dissertation is an original proposal for a comprehensive management and evaluation of postoperative results of intraocular astigmatism correction allowing for the identification of potential causes of refractive surprise in ophthalmology practice.
