Morfologie interiktálních výbojů u fokální kortikální dysplazie

Abstract

Nejčastější příčinou fokální farmakorezistentní epilepsie jsou malformace kortikálního vývoje, konkrétně fokální kortikální dysplazie (FCD). Typ malformace ovlivňuje velikost a charakter epileptogenní léze, jež je cílem epileptochirurgické léčby. Elektrografická aktivita epileptogenní léze může odrážet i její histologický charakter, jehož znalost přispívá k optimální volbě a plánování neurochirurgického výkonu. Cílem práce bylo na základě parametrizace mimozáchvatových epileptiformních výbojů (IED) v předoperačním iEEG pacientů trpících farmakorezistentní epilepsií s FCD lézí rozlišit FCD podtypy a vytvořit klasifikátor, který by umožnil předoperační rozlišení FCD podtypů. Z iEEG záznamů 45 pacientů byly dle dříve publikovaných metod extrahovány IED. IED byly parametrizovány v časové a frekvenční oblasti. Sledované parametry byly statisticky porovnány mezi skupinami pacientů s FCD I a II. Práce prokázala statisticky signifikantní rozdíl mezi délkou trvání IED u FCD I a II během spánku (p = 0,0123). Parametrizace IED umožnila sestavení klasifikátoru schopného rozlišit FCD I a II s až 72\% úspěšností v křížové validaci.

The most common cause of focal pharmacoresistant epilepsy are malformations of cortical development, specifically focal cortical dysplasia (FCD). The type of malformation affects the size and character of the epileptogenic lesion, which is the goal of epilepsy surgical treatment. The electrographic activity of epileptogenic lesion can also reflect its histological character, the knowledge of which contributes to the optimal selection and planning of neurosurgical procedures. The aim of the study was to differentiate FCD subtypes based on the parameterization of interictal epileptiform discharges (IEDs) in preoperative iEEG of patients suffering from pharmacoresistant epilepsy with FCD lesions and to create a classifier that would allow preoperative differentiation of FCD subtypes. IEDs were extracted from the iEEG records of 45 patients according to previously published methods. IEDs were parameterized in the time and frequency domains. The monitored parameters were statistically compared between groups of patients with FCD I and II. The study showed a statistically significant difference between the duration of IEDs in FCD I and II during sleep (p = 0.0123). The parameterization of IEDs enabled the construction of a classifier capable of distinguishing between FCDs I and II with up to 72\% success in cross-validation.
