Korotkovovy zvuky jsou rytmické zázněje, které vznikají při auskultačním měření krevního tlaku v důsledku zaškrcení brachiální arterie. Mechanika jejich geneze ovšem i po takřka sto dvaceti letech výzkumu není stále jednoznačně pochopena. Tato bakalářská práce se věnuje rozboru teorií geneze Korotkovových zvuků, spektrální analýze zaznamenaných zvuků a statistickému porovnání charakteristických parametrů popisující tvar a vlastnosti spekter těchto zvuků mezi dvěma věkovými skupinami (18–23 let a 65–90 let). Metodika vychází z teoretického předpokladu, podle něhož je vznik Korotkovových zvuků zapříčiněn vibrací a rezonancí cévní stěny při rytmickém rozpínaní a hroucení cévy pod tlakem okluzivní manžety. Pružnější a poddajnější cévní struktury by tak měly generovat zvuky s odlišnými vlastnostmi než struktury tužší a nepoddajnější. Korotkovovy zvuky byly zaznamenány během čtyř různých měření krevního tlaku. Pomocí vlnkové transformace byl ze zvuků odstraněn nežádoucí šum a záznamy zvuků byly nasegmentovány na jednotlivé Korotkovovy zvuky. Ty byly následně podrobeny spektrální analýze pomocí Fourierovy transformace a metody Linear predictive coding, spektra jednotlivých zvuků byly popsány charakteristickými parametry, konkrétně čtyřmi spektrálními momenty, mediánem frekvencí spektra a poměrem zastoupení vyšších a nižších frekvencí ve spektru. Na závěr byly rozdíly parametrů mezi věkovými skupinami vyhodnoceny statistickými testy podobnosti a jejich výsledky byly porovnány s teoretickými předpoklady. U metody skokového měření na jednotlivých úrovních tlaku, zejména na úrovních systolického a diastolického tlaku, byly pozorovány význačné rozdíly mezi distribucemi několika parametrů zvuků jednotlivých věkových skupin, což podporuje teorii o souvislosti poddajnosti cévních struktur s vlastnostmi generovaných zvuků. Oproti tomu statistické výsledky z ostatních metod měření naznačují zpravidla shodu v distribucích charakteristických parametrů u obou věkových skupin, což je naopak v rozporu s předpokládanou teorií. Metoda záznamu Korotkovových zvuků při skokovém měření na jednotlivých úrovních tlaku tak přináší potenciální příležitost k dalšímu výzkumu problematiky souvislosti vlastností zvuků a poddajností cévních struktur s ohledem na možnosti diagnostiky aterosklerózy (kornatění cév) a jiných poruch krevního řečiště. The Korotkoff sounds are rhythmic murmurs that arise during auscultatory measurement of blood pressure as a result of brachial artery occlusion. However, the mechanics of their genesis remains incompletely understood after almost a hundred and twenty years of research. The main objective of this bachelor's thesis is the analysis of theories regarding the genesis of the Korotkoff sounds, spectral analysis of recorded sounds and statistical comparison of characteristic parameters describing shapes and properties of spectra of these sounds between two different age groups (18–23 years and 65–90 years). The methodology is based on the theoretical assumption that the production of Korotkoff sounds is caused by vibrations and resonance of the vascular wall during rhythmic expansion and collapse of the vessel under pressure of the occlusive cuff Thus more flexible and yielding vascular structures should generate sounds with different properties than stiffer and more unyielding structures. Korotkoff sounds were recorded during four different blood pressure measurements, unwanted noise was removed from the sounds using the Wavelet transform and the sound recordings were segmented into individual Korotkoff sounds. These were then subjected to spectral analysis employing Fourier transform and Linear Predictive Coding. The spectra of the individual sounds were described with characteristic parameters, namely the four spectral moments, the median of spectrum frequencies, and the ratio of the representation of higher and lower frequencies within the spectrum. Finally, differences in parameters between age groups were evaluated by statistical tests of similarity and the results were then compared with theoretical assumptions. In the method of jump measurements at different pressure levels, especially at systolic and diastolic, significant differences were observed between the distributions of several parameters of the sounds from the two different age groups. This supports the theory of the connection of the compliance of vascular structures with the characteristics of the generated sounds. In contrast, the statistical results from the other measurement methods generally suggest an agreement in the distributions of the characteristic parameters for both age groups, which is contrary to the previously assumed theory. Thus, the method of recording Korotkoff sounds during jump measurements at various pressure levels provides a potential opportunity for further research on the issue of the association between the characteristics of sounds and the compliance of vascular structures, with regard to the possibility of diagnosing atherosclerosis (cornification of blood vessels) and other bloodstream disorders.
