Kriticky nemocní pacienti mají často poruchu distribuce a objemu tělesných tekutin, jedním z projevů je tvorba pleurálního výpotku (pleural effusion, PLE). Podle observačních studií se u 50% až 60% pacientů na umělé plicní ventilaci vyvine pleurální výpotek. Elektrická impedanční tomografie (EIT) je v klinické praxi běžně využívána ke sledování ventilace a účinku léčebných intervencí. Jednou z oblastí, kde lze očekávat nové informace je i zkoumání efektu přítomnosti a odstranění pleurálního výpotku. Odstranění pleurálního výpotku se projeví jako strmý nárůst end-exspirační plicní impedance v oblasti, kde byl lokalizován. Tento vývoj je možné interpretovat jako zvýšení vzdušnosti plicní tkáně dříve utlačené výpotkem. Cílem disertační práce bylo odlišit vliv evakuace PLE od provzdušnění plíce na vývoj impedance a popsat změny v distribuci ventilace, které odstranění PLE způsobí. Současně ověřit, zda je samotné EIT použitelným nástrojem k posouzení vzdušnosti plíce po evakuaci PLE. Na Klinice anesteziologie, resuscitace a intenzivní medicíny 1. Lékařské fakulty University Karlovy a Ústřední vojenské nemocnice v Praze jsme provedli prospektivní studii, která byla zaregistrována v ClinicalTrials.gov pod identifikátorem NCT03231072. Skupina pacientů na umělé plicní ventilaci byla monitorována EIT v období před, během a po evakuaci PLE a současně byl zaznamenáván vývoj end-exspiračního plicního objemu. Na základě provedených měření lze konstatovat, že hlavní příčinou nárůstu impedance bylo odstranění vodivé tekutiny pleurálního výpotku. Z izolovaného záznamu EIT nelze dobře posoudit změny vzdušnosti plicní tkáně po evakuaci pleurálního výpotku. Z hlediska změn v distribuci ventilace je patrné, že roste ventilace v oblasti, kde byl výpotek lokalizován a dochází k celkovému posunu ventilace do dorsálních partií plic. Critically ill patients frequently have impaired distribution and volume of body fluids, one manifestation of which is the formation of pleural effusion (PLE). According to observational studies 50% to 60% of patients on mechanical ventilation develop pleural effusion. Electrical impedance tomography (EIT) is commonly used in clinical practice to monitor ventilation and the effect of treatment interventions. One of the areas where new information can be expected is the investigation of the effect of the presence and removal of pleural effusion. Removal of pleural effusion results in a steep increase of end-expiratory lung impedance in the area where it was located. This development can be interpreted as an increase in the airiness of the lung tissue previously compressed by effusion. The aim of this work was to distinguish the effect of PLE evacuation from lung aeration on impedance development and to describe the changes in ventilation distribution that are caused by PLE removal. At the same time, verify whether the EIT itself is a useful tool to assess air content in lung after PLE evacuation. At the Department of Anaesthesiology, Resuscitation and Intensive Care Medicine, 1st Faculty of Medicine of Charles University and Military University Hospital Prague, we performed a prospective study, which was registered in ClinicalTrials.gov under the identifier NCT03231072. The group of patients on mechanical lung ventilation was monitored by EIT in the period before, during and after the evacuation of PLE, and concurrently the development of end-expiratory lung volume was recorded. Based on the performed measurements, it can be stated that the main cause of the increase in impedance was the removal of the conductive fluid of the pleural effusion. Changes in lung tissue airiness following pleural effusion evacuation cannot be well assessed from an isolated EIT record. In terms of changes in the distribution of ventilation, it is evident that ventilation increases in the area where the effusion was located and there is an overall shift in ventilation to the dorsal parts of the lungs.
