dc.description.abstract | Přesnost teplotních predikcí v povrchové hypertermické léčbě: Cíle: Cílem projektu bylo vyhodnotit přesnost teplotních predikcí pro povrchovou hypertermii v závislosti na změnách teplotních parametrů, a to na prokrvení (perfuzi) a tepelné vodivosti tkáně při hypertermickém ohřevu. Metody: Pro deset pacientů léčených s rekurentním nádorem prsu byly vytvořeny 3D modely ze série CT snímků v programu iSeg. Tyto modely byly skombinovány s modely aplikátorů odpovídající skutečné hypertermické léčbě a pro každou anténu (aplikátor) byl proveden výpočet rozložení elektromagnetického pole. Výsledné pole bylo použito jako zdroj pro teplotní simulace. Pro každý model a čtyři léčby byly porovnány invazivně měřené teploty s predikovanými hodnotami. Dále jsem provedla studii vlivu změny krevní perfuze (prokrvení) a tepelné vodivosti v závislosti na dosažení co nejlepší možné shody měřených a predikovaných teplot. Výsledky: K nejlepší shodě naměřených a predikovaných teplot vedly změny perfuze a tepelné vodivosti (vzhledem k hodnotám pro 37 °C), které se pohybovaly v rozmezí -60 %-176 % pro perfuzi a -98 %-1077 % pro tepelnou vodivost. Medián teploty T50 v léčené oblasti se pohyboval v rozmezí 37,40-47,49 °C. Závěr: Projekt předkládá optimální nastavení simulace změnou hodnoty perfuze a tepelné vodivosti, které nejlépe odpovídá naměřeným teplotám z léčby a ukazuje, jaký vliv má hodnota perfuze a tepelné vodivosti na teplotní rozložení v tkáni v průběhu povrchové hypertermické léčby. | cze |
dc.description.abstract | Accuracy of temperature predictions in superficial hyperthermia treatment: Aims: The purpose of this project was to evaluate accuracy of temperature predictions in superficial hyperthermia. It provides the way the parameters of perfusion and thermal conductivity affect temperature in tissue during hyperthermia heating. Methods: 3D models were created from CT images in iSeg for ten patients treated with recurrent breast cancer. These models were combined with models of applicators corresponding to actual hyperthermic treatment and for each of them the EM-field distribution was calculated. The resulting field was used as a source for temperature simulations. For each model and four treatments, invasively measured temperatures were compared with predicted values. I also performed a study of the effect of changes in blood perfusion (blood circulation) and thermal conductivity depending on achieving the best possible agreement of measured and predicted temperatures. Results: Perfusion changes (relative to that for 37 °C), which ranged from -60 %-176 %, and thermal conductivity changes, which ranged from -98 %-1077 %, led to the best agreement between the measured and predicted temperatures. T50 in the treated area ranged from 37.40-47.49 °C. Conclusions: The project presents optimal setting of the simulation by changing the perfusion and thermal conductivity value, which best corresponds to the measured temperatures from the treatment and shows the influence of perfusion and thermal conductivity value to temperature distribution in tissue. | eng |
