Reconstruction and properties of T2 * parametric maps and their relationship with QSM in patients with REM sleep disorder and Parkinson's disease

Abstract

T2 * relaxačné mapy preukázali zvýšené ukladanie železa v oblasti substantia nigra (SN) u pacientov s Parkinsonovou chorobou (PD). V odborných publikáciách sa objavila zvýšená akumulácia železa aj v iných jadrách hlbokej šedej hmoty, ale výsledky týchto štúdií sú veľmi nekonzistentné. K získaniu presných výsledkov kvantitatívnej analýzy je nevyhnutné pracovať s T2* mapami s vysokou kvalitou bez zbytočného šumu. Cieľom tejto diplomovej práce bolo vytvoriť metodiku na rekonštrukciu T2* parametrických máp z gradient echo (GRE) sekvenčných obrazov a posúdiť vplyv počtu iterácií na ich výslednú kvalitu. Z voľne dostupných programov bol vybraný program ImageJ s nástrojom Mri Processor, v ktorom bola zostavená vhodná metodika. Na posúdenie vplyvu počtu iterácií na kvalitu máp boli použité 3 parametre: krížová korelácia (CS), suma rozdielov štvorcov (SSD) a suma absolútnych rozdielov (SAD). Ako referenčný obraz boli použité mapy s 200 iteráciami. Na základe týchto parametrov boli vyhodnotené mapy so 130 iteráciami ako najvhodnejšie pre kvantitatívnu analýzu v nasledujúcom výskume. Ďalším cieľom práce bolo v rekonštruovaných mapách kvantifikovať množstvo železa v oblasti thalamus (THAL), caudate nucleus (CAUD), putamen (PUTA), globus pallidus (PALL) a substantia nigra (SN) u pacientov s PD (BIOPD), u pacientov s poruchou v REM spánku (RBD) a zdravých kontrol (CON). RBD je považovaná za presymptomatickú fázu PD. Množstvo železa bolo nepriamo kvantifikované pomocou programu FSL. Výsledky kvantitatívnej analýzy odhalili významnú akumuláciu železa v oblasti PUTA u skupiny RBD (ANCOVA: p = 0,028) a v oblasti SN u skupiny BIOPD (WT: p = 0,049). Posledným cieľom práce bola korelácia výsledných R2* (1/T2*) hodnôt s hodnotami magnetickej susceptibility z QSM máp, ktoré boli rekonštruované z rovnakých GRE sekvenčných obrazov. Výsledky korelačnej analýzy odhalili silnú koreláciu pre oblasť PUTA (? = 0,78; p < 0,01) a strednú koreláciu pre oblasti CAUD (? = 0,61; p < 0,01) a PALL (? = 0,58; p < 0,01). Korelácia pre oblasť SN (? = 0,32; p = 0,02) sa javila ako slabá a výsledné hodnoty pre oblasť THAL (? = 0,00; p = 0,99) spolu vôbec nekorelovali. Výsledkom tejto práce bolo vytvorenie vhodnej metodiky na rekonštrukciu T2* parametrických máp, ktoré je možné následne použiť pri kvantitatívnej analýze železa u PD. Korelácia výsledných R2*(1/T2*) hodnôt s hodnotami magnetickej susceptibility z QSM máp u rovnakých probandov umožnila potvrdiť alebo vyvrátiť dôveryhodnosť výsledkov kvantitatívnej analýzy.

T2* mapping has demonstrated an increased iron deposition in the region of substantia nigra (SN) in the patients with Parkinson's disease (PD). An increased accumulation of iron was according to some scientific studies also found in other subcortical deep grey matter nuclei, but the results of these findings were not consistent. The high quality of T2* maps without useless noise is necessary for achieving accurate results of quantitative analysis. The aim of this diploma thesis is to develop a methodology for the reconstruction of the T2 * parametric maps from the gradient echo (GRE) sequences and to evaluate the impact of the number of iterations on the quality of maps. From the available tools the program ImageJ with Mri Processor tool was selected and an appropriate methodology was created. To assess the effect of the number of iterations three parameters were used: cross-correlation (CS), sum of square differences (SSD) and sum of absolute differences (SAD). The map with 200 iterations was chosen as a reference image. Based on these parameters, the maps with 130 iterations were evaluated as the most suitable ones for the quantitative analysis in the next research. Another aim of this thesis was to quantify the amount of iron in the reconstructed maps in the region of thalamus (THAL), caudate nucleus (CAUD), putamen (PUTA), globus pallidus (PALL) and substantia nigra (SN) in the patients with PD (BIOPD), in the patients with REM sleep disorder (RBD) and healthy controls (CON). RBD is considered as a presymptomatic stage of PD. The content of iron was indirectly measured by using the FSL program. The results of the quantitative analysis revealed a significant accumulation of iron in the PUTA region in the RBD group (ANCOVA: p = 0,028) and in the SN region in the BIOPD group (ANCOVA: p = 0,049). The last aim of the thesis was the correlation of the final R2* (1/T2*) values with the values of magnetic susceptibility from QSM maps, which were reconstructed from the same GRE sequences. The results of the correlation analysis revealed a strong correlation for the PUTA region (? = 0.78; p < 0.01) and a mean correlation for the CAUD (? = 0.61; p < 0.01), and PALL (? = 0.58); p < 0.01) region. The correlation for the SN region (? = 0.32; p = 0.02) was weak and the resulting values for the THAL region (? = 0.00; p = 0.99) weren´t correlated at all. The result of this work is the creation of an appropriate methodology for the reconstruction of T2 * parametric maps, which can be used in the quantitative analysis of iron in PD. The correlation analysis of final R2*(1/T2*) values with magnetic susceptibility values from QSM maps for the same probands enables to confirm or refuse the precision of the results.
