Efekty a nákladová efektivita včasné rehabilitace pacientů po cévní mozkové příhodě

Abstract

Práce se zabývá problematikou včasné rehabilitace pacientů po cévní mozkové příhodě. Cílem práce je na základě dat získaných z proběhlé klinické studie provést analýzu nákladové efektivity ve třech českých iktových centrech zapojených do studie a analyzovat možnosti predikce stavu pacientů jeden rok po propuštění z nemocnice. Výsledky práce prokázaly významné zlepšení pacientů v motorické části testu FIM rok po propuštění z nemocnice, naopak zlepšení v kognitivní části testu FIM se u souboru pacientů neprokázalo. Dále nebyl prokázán přímý vliv věku, délky hospitalizace ani pohlaví pacientů na jejich zlepšení po roce v testu FIM. Potenciálním ovlivňujícím faktorem byl shledán stupeň postižení pacientů při přijetí, kdy těžší postižení predikovalo výraznější zlepšení pacientů jeden rok po propuštění z nemocnice. Výsledky analýzy nákladové efektivity pro jednotlivé kategorie pacientů v ústavní péči ukázaly, že vyšší stupeň postižení pacientů je spojen s nárůstem průměrných nákladů na rehabilitační léčbu, avšak průměrné inkrementální náklady na zlepšení klinického efektu o jeden bod ve skóre FIM se s rostoucí mírou postižení nezvyšují.The thesis deals with the problems of early rehabilitation after stroke. Based on data gained from a clinical trial, the aim of the thesis is to analyze cost effectiveness in three Czech stroke centers involved in the trial, and to analyze possibilities of prediction of patient state one year after hospital discharge. Results of the analysis showed significant improvement of patients in the motor part of the FIM test one year after discharge, improvement in the cognitive part of the FIM test wasn't proved. Furthermore, any effect of patients' age, sex and duration of hospitalization on the improvement after one year in the FIM test did not manifest itself. The degree of patients' disability upon hospital admission was found to be a potentially influencing factor where more severe disability predicted greater improvement of patients one year after the discharge. Results of the cost effectiveness analysis for individual categories of patients in institutional care showed that a higher degree of patients' disability is connected with the increase in average rehabilitation care costs; however, average incremental costs of improvement of the clinical effect by one FIM point did not increase along with a higher degree of disability.
