Porovnání nákladů u dvou rozdílných typů stabilizačních operací bederní páteře

Abstract

Cílem diplomové práce je vyčíslení skutečných nákladů na miniinvazivní operační techniku degenerativního onemocnění bederní páteře technikou MIDLF a následné porovnání zjištěných nákladů s náklady na klasickou otevřenou stabilizaci bederní páteře. Dílčími cíli jsou komparativní porovnání těchto dvou operačních technik a porovnání skutečných nákladů na miniinvazivní operační techniku degenerativního onemocnění bederní páteře technikou MIDLF s úhradami od zdravotních pojišťoven. Základem diplomové práce je analýza současného stavu dané problematiky v České republice a ve světě. Dále pak identifikace vhodných kalkulačních metod nákladů ve zdravotnictví. Analýza je provedena na základě systematické rešerše zaměřené na vybrané techniky stabilizační operace bederní páteře a také na identifikaci vhodných kalkulačních metod. Na základě analýzy současného stavu problematiky byla zjištěna absence ekonomické analýzy nákladů na zvolenou operační techniku MIDLF, která je provedena v diplomové práci. Po prostudování současného stavu problematiky vhodných kalkulačních metod ve zdravotnictví byla pro účely diplomové práce zvolena kalkulace Activity-Based Costing. Za pomoci této kalkulace a komparační metody bylo možné zpracovat vytyčené cíle diplomové práce, čili stanovit skutečné náklady na miniinvazivní operační techniku degenerativního onemocnění bederní páteře MIDLF a tyto náklady porovnat s náklady na klasickou otevřenou stabilizaci bederní páteře. Skutečné náklady techniky MIDLF byly následně srovnány se skutečnými úhradami od zdravotních pojišťoven. V neposlední řadě došlo ke komparativnímu porovnání těchto dvou metod.The aim of this diploma thesis is to quantify the real costs of a mini-invasive surgery technique of the degenerative diseases of lumbar spine by the MIDLF technique and to compare established price to costs of classical open lumbar spinal stabilization. Partial objectives are the comparison of these two operating techniques and the comparison of real costs of mini-invasive operating technique of the degenerative disease of lumbar spine by the MIDLF technique with health insurance compensation. The basis of this thesis is the current state analysis of this issue in the Czech Republic and worldwide. Furthermore, there is the identification of suitable cost calculation methods in the health care. The analysis is based on systematic research, which is focused on chosen techniques of lumbar spine stabilization operating and identification of suitable calculation methods. Based on the current state analysis, the absence of economic cost analysis of the chosen MIDLF operating technique is discovered in the thesis. After studying the status quo of suitable calculation methods in the health care, the Activity-Based Costing calculation was selected for the purpose of this thesis. With the help of this calculation and comparative method, I was able to process the given objectives of the thesis that means to set the real costs of mini-invasive operating technique of the degenerative disease of lumbar spine MIDLF and compare these costs to costs of classical open lumbar spine stabilization. The real costs of MIDLF technique are also compared to real payments of the health insurance companies. Finally, there is a comparison of these two methods.
